| Objective: To explore the relationship between the end plate defect of lumbar vertebrae and lumbago and disc degeneration.Methods: From March 2014 to August2018,the clinical data of 87 patients with vertebral endplate defects treated in our orthopaedic center due to lumbar diseases were analyzed retrospectively.According to the rajasekaranet grading method,the vertebral endplate defects were classified into gradeⅠ ~Ⅵ.The age,BMI,VAS score,JOA score and ODI dysfunction score of each patient were recorded and evaluated.The improved Pfirrmann score was used The severity of degeneration was divided into 1-5 points according to the grade standard.Finally,SPSS software package was used to analyze the correlation between the vertebral endplate defect and these indexes.Result:Among the 87 patients,according to rajasekaranet grade I,there were 15 patients,13 patients of grade II,11 patients of grade III,16 patients of grade IV,19 patients of grade V and 13 patients of grade VI.The mean age was 47.3 years± 13.1 years(21-78 years);the mean BMI was 22.4 ± 3.7 years(17-33 years);the mean VAS score was 0.98 ± 1.0(0-5 points);the mean JOA score was 24.4 ± 4.4(9-29 points);the mean ODI dysfunction score was 6.9% ± 6.5%(0-30.2%);the mean disc degeneration score was 2.2 ± 1.3(1-5 points).16 cases(18.4%)had Modic changes in the vertebral endplate.There was a positive correlation between vertebral endplate defect and VAS score(r = 0.765,P < 0.001),but a negative correlation with JOA score(r =-0.685,P <0.001).When the vertebral endplate defect was in grade I and II,there was no correlation between the vertebral endplate defect and disc degeneration(r = 0.212,P = 0.279);while when the vertebral endplate defect was in grade III or above,there was a positive correlation between the vertebral endplate defect and disc degeneration(r = 0.770,P <0.001).Conclusion: There was a significant correlation between the end plate defects of lumbar vertebrae and lumbago and disc degeneration(≥ grade III). |
