Objective: the purpose of this study was to further understand or observe the efficacy of dipeptidyl peptidase-4(DPP-4)sioglitine and GLP-1 receptor agonist liralutide in the treatment of microalbuminuria in patients with early type 2 diabetic nephropathy,and to further analyze the risk factors associated with the progression of diabetic nephropathy.Methods: Patients were randomly assigned to three groups of patients: basal insulin treatment group,insulin + sitagliptin treatment group,and insulin +liraglutide treatment group.After 24 weeks of intervention,relevant parameters were determined before and after the intervention.Analysis and evaluation,of which UACR is the main outcome indicator.Results: 1.Comparison of related parameters of renal filtration function before and after intervention: UACR of the three groups of patients all decreased significantly(P <0.05),but compared between groups,sitagliptin group was superior to insulin treatment group(P <0.005),while liraglutide was superior to the sitagliptin group and the insulin group,the difference was significant(P <0.001).2.FPG,PPG and HbA1 c were significantly decreased before and after the experiment(P <0.001).The liraglutide group was significantly better than the other two groups(P <0.05).3.Comparison of blood lipid,blood pressure,body weight and body mass index before and after the experiment: the liraglutide group was better than the sitagliptin group in terms of weight and BMI improvement,the difference was significant(P <0.01).4.Take UACR as the dependent variable and carry out multiple linear regression analysis.The main factors affecting UACR include the length of medical history,body mass index and systolic blood pressure,which are positively correlated.Conclusions: 1.In the improvement of early DKD lesions combined with microalbuminuria,it is suggested that liraglutide has more advantages than sitagliptin.2.Liraglutide and sitagliptin both have different degreesof weight loss,lower blood fat,restore pancreatic islet function,lower systolic blood pressure and other hypoglycemic effects.Liraglutide shows more obvious advantages than other two groups The risk factors of DKD are closely related to the course of disease,BMI and SBP.
