Objective: to analyze the clinical characteristics and therapeutic effect of brain metastasis of lung adenocarcinoma,and to explore the prognostic factors of brain metastasis of lung adenocarcinoma.Materials and methods:from January 2015 to January 2019,98 patients with brain metastasis of lung adenocarcinoma with EGFR mutation were selected from the Third Clinical Medical College Affiliated to Xinjiang Medical University.Age,gender,kappa’s score,mutation type,presence or absence of brain metastasis symptoms,number of brain metastasis tumors,meningeal metastasis,extracranial metastasis,primary focus control and treatment were analyzed Objective: To investigate the prognostic factors of patients with brain metastasis.Results: 1.All the 98 patients with brain metastasis of lung adenocarcinoma could evaluate the curative effect.The control of primary brain metastasis,the number of intracranial metastasis,the score of patients,the extracranial metastasis and the treatment mode could affect the prognosis of patients(P < 0.05);2.The results of multi factor analysis were the control of primary brain metastasis,the short-term curative effect of extracranial metastasis and intracranial metastasis,and the treatment mode was to affect the brain metastasis of lung adenocarcinoma The independent factors of prognosis in patients with metastasis(P < 0.05).The prognosis of patients with KPS score greater than or equal to 80,stable primary lung lesions in brain metastasis and patients without extracranial metastasis were better than that of the control group,with statistical analysis difference(P < 0.05).3.The PFS of EGFR-TKIs,EGFR-TKIs and EGFR-TKIs were superior to chemotherapy combined radiotherapy(15 months vs 12 months vs 7 months vs3 months,P = 0.007).The difference was statistically significant.OS of EGFR-TKI synchronous radiotherapy group,EGFR-TKI single drug targeted therapy group and EGFR-TKI sequential radiotherapy group were better than that of chemotherapy combined radiotherapy group(26.3 months vs 22.3months vs 20 months vs 13.7 months,P = 0.010).Conclusion: 1.The prognosis of patients with brain metastasis of lung cancer is affected by kappa score,the control of primary lung focus during brain metastasis,and whether there is extracranial metastasis.On the contrary,there is no significant correlation between age,gender,smoking and the number of metastases.2.2.The treatment of brain metastasis of lung adenocarcinoma will affect the short-term effect and median survival time of the patients.The PFS and OS of EGFR-TKIs,egfr-tks alone and EGFR-TKIs sequential radiotherapy were better than those of non TKIs.
