| In recent years,the state has been promoting the construction of a healthy China,which guides residents to go to the grassroots to see a doctor,and form a scientific medical treatment system to become the focus of policy.Although the state has increased the proportion of reimbursement for primary health care institutions and guided residents to go to the grassroots level,the primary medical institutions have not changed their medical treatment rate or hospital income,and there are even signs of retrogression.In theory,the proportion of medical insurance payment can affect the choice of medical treatment for residents,but the reality has little effect.When the patient chooses to visit,not only the price and quality of the medical service are the main factors affecting the patient.There is information asymmetry in the medical service market,and reputation has become a signal for transmitting medical service quality information.This paper introduces reputation,which reduces the transaction cost of patients’ access to medical service quality information and is the main influencing factor for patients’ choice of treatment.This paper finds that reputation has a positive effect on the patient’s choice of medical treatment.When the primary medical institutions have reimbursement advantages but the reputation is not good,the increase in the proportion of reimbursement of primary medical institutions cannot compensate for the decline in their reputation.The quality of medical care per unit cost is relatively low,and the possibility of patients at the grassroots level is reduced..This explains the reasons for the failure of the reimbursement policy of existing primary health care institutions.There is a difference in reputation between primary health care institutions and large medical institutions.Through the establishment of a reputation model,this paper finds that the income,initial reputation,the rate of exchange,and the discount rate of medical institutions have a positive effect on its reputation.However,the reputation mechanism is not fully functional.The government’s price control has weakened the hospital’s income.The current medical institution management model has weakened the actual effect of spot checks,and the vague reputation property rights have reduced the discount of future income,which has adversely affected the reputation mechanism.This paper suggests introducing a competitive mechanism to increase patient choices;changing government functions to help disseminate information about primary health care services;promoting free practice and clarifying the reputation rights of primary care products. |
