Objective:Finding the description of recurrent oral ulcer in traditional Chinese medicine by looking through the ancient literature, study the etiology and pathology of dialectical recurrent oral ulcer, and dialectically treat it;through clinical investigation of clinically relevant risk factors of recurrent oral ulcer, and analyze its main correlation by statistical analysis The risk factors provide a better basis for the clinical prevention and clinical dialectical treatment of recurrent oral ulcersMethod:Review the classic books of ancient Chinese medicine, collect the classic descriptions of aphthous ulcers(recurrent oral ulcers)in Chinese medicine, and the descriptions of other oral mucosal diseases and other dental diseases by traditional Chinese medicine, and summarize the etiology and pathogenesis of oral ulcers and dialectics Governance awareness. The self-made ` Recurrent Oral Ulcer-related Pathogenic Factors Questionnaire ’ was used for newly diagnosed patients of all ages at the ROU of the Department of Stomatology of Hubei Maternal and Child Health Hospital and Hubei Provincial Hospital of Stomatology from November 2018 to January 2020,and the same period Patients with non-oral mucosal disease who visited the department of dentistry of the two hospitals filled out questionnaires, analyzed single factors by chi-square test, and used logistic regression analysis to identify the risk factors that caused clinical occurrence of ROU;Collect the four diagnosis data of patients for dialectical classification, and provide basis for dialectical treatment.Results:1.The classics of traditional Chinese medicine, including "The Yellow Emperor’s Canon",have described the causes and pathogenesis of aphthous ulcers, and dialectical treatment, and there is a lot of discussion on other oral mucosal diseases and dental diseases in Chinese medicine.2.According to the statistical analysis of the questionnaire, there were 88 males in the case group, with an average age of 40.02± 16.058 years and 109 females, with an average age of 38.19 ±14.924 years. There were 90 males in the control group, with an average age of 39.09 ± 17.203 years and 110 females, with an average age of 36.97 ± 13.807 years. There was no difference in the composition of age and gender between the two groups.3.The incidence of newly diagnosed patients in ROU was: lip(61.421%),tongue(47.716%),cheek(37.563%),palate(8.122%),gums(5.076%),vestibular sulcus(2.031%),and floor of mouth(0.508).%);The duration of disease was more common than 24 months, accounting for 46.701% of the total number;172 cases(87.307%)of mild type,7 cases(3.551%)of severe type,18 cases of herpes type(9.142%)4.The results of single factor chi-square test of clinical pathogenic factors of ROU are as follows: history of gastrointestinal diseases(P = 0.000),family history(P = 0.000),excessive drinking(P = 0.000),preference for spicy food(P =0.006),Like to eat beef and mutton(P = 0.000),preserved food(P = 0.000),tiredness(P = 0.000),irritability(P = 0.000),the difference has statistical significance(P <0.05).The results of multivariate Logistic regression analysis are as follows: history of digestive tract disease, family history, excessive alcohol consumption, preserved food, fatigue, and irritability. The OR values were 1.859(P = 0.006),2.003(P = 0.005),and 1.725(P =0.019),1.815(P = 0.008),1.893(P = 0.004),1.902(P = 0.003),and the differences were statistically significant(P <0.05).5.The frequency of TCM symptoms of the newly diagnosed patients in ROU from the largest to the smallest are: pain, red tongue, hot heart, yellow greasy fur,and constipation.TCM syndrome differentiation results for ROU patients: 149 cases of heart and spleen fever syndrome(75.635%),28 cases of spleen and kidney yang deficiency syndrome(14.213%),20 cases of yin deficiency and fire deficiency syndrome(10.152%).Conclusion:1.Chinese medicine has a long history of theoretical understanding of aphthous ulcers and various dental diseases, and put forward corresponding treatment methods. It was one of the most advanced theories in the world at the time, but subsequent theoretical and practical developments failed to follow. The trend of the times.2.ROU patients are more common in young and middle-aged women. The course of disease is usually more than 2 years. The most common sites are lip, tongue, and buccal mucosa. The incidence is mainly light.3.History of gastrointestinal diseases, family history, excessive alcohol consumption, spicy food preferences, beef and mutton preferences, preserved foods, tiredness, and irritability are all related to the occurrence of ROU.The order of risk according to size is: family history, irritability,Fatigue, history of digestive tract diseases, preserved foods, excessive drinking.4.The frequency of TCM syndromes of newly diagnosed patients in ROU in descending order is: pain, red tongue, irritable heart, yellow greasy fur,and constipation. The syndrome differentiation of TCM is based on the accumulation of heart and spleen fever. And herpes-type ROU and severe ROU are dialectical for heart and spleen fever.
