| Objective:This study intends to design an assisted syndrome differentiation model for cough disease in Chinese medicine and verify its agreement with expert diagnosis.It can be applied to the clinical teaching of cough disease in TCM and the development of an assisted syndrome diagnosis system.Also,it can assist in the syndrome differentiation of clinical doctors.Methods:This study is divided into two parts.The first part is the construction of an assisted syndrome differentiation diagnosis model.The model is based on the "Traditional Chinese Medicine Syndrome Diagnosis and Efficacy Standards-Cough"(ZY/T001.1-94)and"Traditional Chinese Medicine Internal Medicine Clinical Diagnosis and Treatment Guidelines-Cough "(ZYYXH/T4-2008),the fifth edition of Traditional Chinese Medicine",based on the cough part,and then search the" CNKI ","Wanfang","VIP","Duxiu" databases related to cough Symptoms and clinical manifestations of the literature,after screening the statistics of cough disease syndrome elements(hereafter referred to as syndromes)and the frequency of clinical manifestations,and calculating the proportion of syndromes and clinical manifestations,to obtain higher frequency of syndromes and Clinical manifestations.Refer to"Traditional Chinese Medicine Clinical Common Terms Glossary" for standardization of clinical manifestations,and finally refer to "Traditional Chinese Medicine Pathogenesis Syndrome","Syndrome Differentiation" and combine the theory of organic resonance theory to construct each syndrome Diagnostic expression.In the second part,102 clinical cases of cough were collected,and the coincidence rate of model diagnosis with artificial initial diagnosis and expert diagnosis was analyzed.result:1.Based on "Traditional Chinese Medicine Syndrome Diagnosis and Efficacy Standards-Cough.(ZY/T001.1-94),"Traditional Chinese Medicine Internal Medicine Clinical Diagnosis and Treatment Guidelines-Cough"(ZYYXH/T4-2008),and the fifth edition of"Traditional Chinese Medicine",a cough disease-assisted syndrome diagnosis model that can be directly applied to software development is constructed.2.Case verification statistics show that the most frequent symptoms of cough are phlegm dampness,heat,and wind,and the lowest frequencies are dryness,yang deficiency,kidney deficiency,and stasis.3.For the diagnosis of a single syndrome:the average accuracy of the model and the expert is 84.05%,the average accuracy of the fuzzy is 98.22%,and the artificial and the expert:the average of the accuracy is 67.74%,and the average of the fuzzy is 90.37%.The artificial diagnosis of the anastomosis McNemar test shows that in addition to the exact anastomosis of cold,the fuzzy anastomosis of yin deficiency and qi deficiency,the precise and fuzzy anastomosis of liver fire,yang deficiency,kidney deficiency,and stasis,the other results show statistical differences,indicating that in these cases Model diagnosis has higher agreement than manual diagnosis.4.Diagnosing with combined syndromes:model and expert:exact agreement 42.16%,overall agreement 20.59%,fuzzy agreement 82.35%,artificial diagnosis and expert:precise agreement 21.57%,overall agreement 2.94%,fuzzy agreement The degree was 35.29%.The McNemar test values of the model and artificial diagnosis coincided with each other were less than 0.05,which was statistically different.Conclusion:1.It is verified that the coincidence rate between the diagnosis of this model and that of expert diagnosis is better than that of artificial initial diagnosis.Combined with its clear logical connotation,it can be directly applied to software development and has practical application value.2.Thanks to its clear logical connotation,this model has certain significance in the teaching and training of junior doctors of traditional Chinese medicine physicians or western medicine physicians,and is the goal of continuing research. |
