Study On TCM Syndromes Elements And Related Factors Of Insomnia With Anxiety

ObjectiveThe purpose of this study is mainly to investigate the distribution of syndrome elements in insomnia patients with anxiety,to summarize the common syndrome types,and to study the correlation between the distribution of syndrome elements and the anxiety degree and sleep quality of patients,so as to make the TCM syndrome differentiation of insomnia with anxiety more objective and standardized,and to provide a theoretical basis for the syndrome differentiation and treatment of this disease.Methods101 insomnia with anxiety patients who met the inclusion criteria were selected form outpatient and ward department of cephalopathy center in Jiangsu Provincial Hospital of Traditional Chinese Medicine form April 2019 to January 2020.Basic information and the four diagnosis information of TCM of patients were collected to establish a database.Referencing Syndrome Elements Differentiation by Professor Zhu Wenfeng’s,this paper analyzes the four diagnostic information of traditional Chinese medicine,sorts out the relevant syndrome information and recorded in the database.Kendall’s,Spearman’s correlation test,chi square test and cluster analysis in Spss22.0 were used to analyze the basic information and related syndrome factors of patients,.ResultThe results showed that there were 9 location syndrome elements including liver,heart,kidney,spleen,stomach,uterus,heart,meridians and surface.The liver syndrome element accounted for the highest proportion(88.12%),followed by heart spirit syndrome element,which was as high as 57.43%.Other location syndrome elements also involved kidney,spleen,stomach and uterus,which were accounting for 36.63%,24.75%,16.83%and 11.88%respectively,and the rest were less than 10%.There are 16 pathogenic syndrome elements,the most of which are There were 88 cases of qi stagnation and 88 cases of yin deficiency among 16 pathogenic syndrome elements,accounting for 87.13%.Other pathogenic syndrome elements including 76 cases of yang hyperactivity,58 cases of heat,54 cases of qi deficiency,51 cases of dampness,45 cases of blood deficiency,30 cases of phlegm and 19 cases of blood stasis account for 75.25%,57.43%,53.47%,50.50%,44.55%,29.7%and 18.81%respectively.Among the location syndrome elements,the proportion of three disease position syndrome element combination is the most,as high as 32.67%,in which the combination of liver and mind is the highest,as high as 18.81%,followed by three disease position syndrome element combination(22.77%),single disease position(18.81%),four disease position syndrome element combination(15.84%).And the syndrome element combination of four disease positions is the more diverse,with eleven combinations.Among the combination of pathogenic syndrome elements,the number of cases with five disease syndrome element combinations was the most(28.71%),followed by six disease syndrome element combinations(26.73%),four disease combinations(16.83%),among which the highest proportion is heat,yin deficiency,yang hyperactivity and qi stagnation,occuring seven times.Main TCM syndromes are deficiency of liver yin,hyperactivity of liver Yang,stagnation of liver qi,stagnation of liver qi and fire,stagnation of uterus with deficiency of spleen and stomach,deficiency of Qi and blood with deficiency of kidney and uneasiness.The distribution of pathogenic syndrome elements and location syndrome elements is strongly associated with HAMA scors,while having little to do with other factors.The distribution of mind,qi deficiency and heat syndrome are more common in PSQI score(6-10).ConclusionThe results showed that the main location syndrome elements of insomnia with anxiety liver and mind,involving kidney,spleen,stomach and uterus.The main pathogenic syndrome elements are qi stagnation,yin deficiency,yang hyperactivity,heat,qi deficiency,dampness,blood deficiency,phlegm and blood stasis,among which the highest proportion are qi stagnation and yin deficiency.Disease location syndrome elements combination is mainly double disease,among which the combination of liver and mind occupies higher proportion.The pathogenic syndrome elements combination are diversified.Among the combination of pathogenic syndrome elements,five disease syndrome element combination contains the most number of cases,and the combination of heat,yin deficiency,yang hyperactivity,qi stagnation combination is the most common.Main TCM syndromes are deficiency of liver yin,hyperactivity of liver Yang,stagnation of liver qi,stagnation of liver qi and fire,stagnation of uterus with deficiency of spleen and stomach,deficiency of Qi and blood with deficiency of kidney and uneasiness.The more serious the anxiety level is,the more number of location syndrome elements and pathogenic syndrome elements are involved.The distribution of mind,qi deficiency and heat syndrome are more common in PSQI score(6-10).
