Changes And Significance Of Serum VEGF Levels Before And After Radioactivity Iodine Treatment For The Patients With Differentiated Thyroid Cancer

Objective:Thyroid cancer is the most common type of tumor in the endocrine system,and differentiated thyroid carcinoma(DTC)accounts for more than 90% of all thyroid tumors.The generation of tumor blood vessels is an indispensable way for tumor growth,invasion and metastasis.Vascular endothelial growth factor(VEGF)is one of the important angiogenic factors involved in the growth of normal and tumor tissues.Studies have shown that tumor recurrence is related to tumor angiogenesis,and VGEF is closely related to the angiogenesis,growth and invasion of thyroid cancer.Therefore,the purpose of this study was to investigate the changes in serum VEGF levels and their significance before and after the first postoperative treatment with radioactive iodine for differentiated thyroid carcinoma.Methods: A total of 78 DTC patients(73 papillary carcinoma and 5 follicular carcinoma)with complete clinical data were collected from the department of nuclear medicine,the first affiliated hospital of wannan medical college from October 2018 to January 2020 who were first treated with radioactive iodine.There were 29 males and 49 females,ranging in age from 19 to 65 years,with an average age of 40.7 years.All patients with DTC had undergone total or subtotal thyroidectomy and were confirmed as DTC by pathology,and the relevant examination showed that they met the treatment conditions of iodine.After consultation,imaging and laboratory tests,all the patients were excluded from related diseases affecting serum VEGF levels,such as diabetes,coronary heart disease,hypertension,heart failure,chronic obstructive pulmonary disease,autoimmunediseases,rheumatoid arthritis,platelet abnormalities,other tumors,and pregnant and lactating women.Serum VEGF levels were determined by enzyme linked immunosorbent assay(ELISA)prior to the treatment of radioactive iodine,and the indexes were re-examined 1 and 3 months after the treatment of radioactive iodine.Finally,statistical methods were used to analyze and compare the differences in serum VEGF levels before and after the treatment of radioactive iodine and the first and third months after the treatment of radioactive iodine.Results:(1)Serum VEGF levels of DTC patients were 394.12±55.56pg/ml before radioactive iodine treatment,373.70±44.21pg/ml and 360.01±49.23pg/ml at the 1st and3 rd months after radioactive iodine treatment,respectively.The serum VEGF level at the1 st month after the treatment of radioactive iodine was significantly lower than that before the treatment of radioactive iodine,and the difference was statistically significant(P=0.000<0.05);The serum VEGF level at 3 months after the treatment of radioactive iodine was significantly lower than that before the treatment of radioactive iodine,and the difference was statistically significant(P=0.000<0.05);The serum VEGF level at the 3rd month after the treatment of radioactive iodine was significantly lower than that at the 1st month after the treatment of radioactive iodine,and the difference was statistically significant(P=0.013 < 0.05).(2)There was no significant difference in serum VEGF levels between different genders,ages and lesion locations before and after the treatment of radioactive iodine,but there were significant differences in risk of recurrence,pathological types and presence or absence of thyroid tissue residues.(3)The results of multiple linear regression analysis showed that the risk of recurrence of DTC after surgery,pathological type and presence of thyroid tissue residue were the key factors affecting the serum VEGF level.Conclusion:(1)The serum VEGF level of DTC patients after radioactive iodine treatment was significantly lower than that before treatment,indicating that detection of serum VEGF level was of certain guiding significance in assessing the changes in the conditionof DTC patients after radioactive iodine treatment.(2)Postoperative serum VEGF level of DTC was correlated with pathological type,recurrence risk and presence of residual thyroid tissue,and was independent of gender,age and lesion location.
