Objective By analyzing the age,gestational week,region,education level,hematuria biochemical test results,ultrasound examination results,treatment methods,and pregnancy outcomes of pregnant patients with symptomatic urolithiasis,the influencing factors and symptoms of surgical decision-making in pregnancy with symptomatic urolithiasis High risk factors for urolithiasis and adverse pregnancy outcomes of pregnancy combined with symptomatic urolithiasis,in order to provide individualized prevention of urolithiasis in pregnant population,and provide scientific theoretical basis for clinical front-line personnel in the process of diagnosis and treatment And reference value.Methods The data of 61141 in-patient pregnant women admitted to the First People’s Hospital of Chenzhou from January 2006 to December 2018 were collected,and 176 cases of pregnancy-associated symptomatic urolithiasis were screened and the symptomatic urine were recorded.Observation indicators such as age,education,region,pregnancy,clinical symptoms,hematuria biochemical test,urinary system ultrasound examination,treatment method,and other observational indicators;follow-up records of recurrence,prognosis,and pregnancy outcome.Results(1)184 cases of 61141 pregnant women were diagnosed with urolithiasis,the incidence rate was 0.3%,of which 176 cases weresymptomatic urolithiasis,the incidence rate was 0.29%,and the incidence rate was relatively high in the age group> 40 years old.It accounts for 0.97%;the incidence rate is higher at 28 to 36 weeks of gestation,accounting for 44.32%;the top three areas for the incidence of pregnancy-associated symptomatic urolithiasis in Luzhou City are Chenzhou City(42.61%)and Guiyang County(12.50)%),Yongxing County(11.36%);those with pregnancy-associated symptomatic urolithiasis in Chenzhou area had a relatively high education level of high school and below,accounting for 66.48%.(2)Leukocyte count,serum creatinine,urinary protein,uroketone body,urinary white blood cells,urinary glucose,and urinary red blood cells were significantly higher in pregnant patients with symptomatic urolithiasis than in pregnant women without urolithiasis(P <0.05).In the analysis of blood and urine biochemical abnormalities,patients with pregnancy-associated symptomatic urolithiasis had increased white blood cell counts,urinary leukocytes,urinary erythrocytes,urinary protein,urinary glucose,urinary ketones,and abnormalities in the history of urolithiasis before pregnancy The rate was significantly higher than in pregnant patients with asymptomatic urolithiasis.(3)There were significant differences in the treatment methods for patients with pregnancy-associated symptomatic urolithiasis with or without nausea and vomiting,lower urinary tract symptoms,microscopic pyuria,and abnormal serum creatinine(P <0.05).(4)There are significant differences in the treatment methods for the stone position,diameter,and degree of hydronephrosis in the ultrasound examination results of pregnant women during pregnancy(P <0.05);And severe hydronephrosis may be an independent risk factor for surgical intervention(P <0.05).(5)The adverse pregnancy outcomes of premature delivery and cesarean section are significantly different between the pregnant complication group with symptomatic urolithiasisand the group without pregnant women with urolithiasis.The outcome was significantly higher than that of pregnant women without urolithiasis(P <0.05).Conclusions(1)The overall prevalence of pregnant women with symptomatic urolithiasis in Chenzhou is 0.29%,and the prevalence of pregnancy with symptomatic urolithiasis in the age group> 40 years is relatively high.(2)Elevated blood creatinine,accompanied by moderate to severe hydronephrosis,and late pregnancy may be independent influencing factors for surgical intervention in pregnant women with symptomatic urolithiasis.(3)Pre-pregnancy history of urolithiasis,increased white blood cell count,increased blood creatinine,positive white blood cells in urine,positive red blood cells,positive urine glucose,positive urine protein,positive urine ketone body may cause pregnancy with symptomatic urolithiasis relevant factors.(4)Among pregnant women with symptomatic urolithiasis,there is an increased risk of premature delivery and cesarean section.
