Objectives:To investigate the changes and relationships between serum cystatin C and urine albumin-creatinin ration(UACR)levels in patients with cerebral infarction during the acute phase,and the relationship between the two and the National Health Research Correlation of All Stroke Scores(NIHSS).Methods:100 patients with acute cerebral infarction who were hospitalized in the Department of Neurology,Dalian Municipal Central Hospital from January to November 2019 were included as the ischemic group.80 non-cerebral infarction patients during the same period were included as the control group.We collected general clinical data of patients in the experimental group and the control group,including age,gender,hypertension,diabetes,coronary heart disease,and carotid arteriosclerosis,and conduct detailed examination and consultation on the experimental group on the day of admission.Neurological impairment was assessed using the National Institutes of Health Stroke Scores(NIHSS).All subjects were collected venous blood on an empty stomach early in the morning after admission,then tested total cholesterol(TC),triglyceride(TG),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),lipoprotein(a)(Lpa),homocysteine(Hcy),and cystatin C levels,and detection of protein / creatinine ratio(UACR)levels.SPSS 24.0 software was used for statistical analysis of the data,using t-test,chi-square test,Logistic regression analysis,Pearson correlation coefficient and other statistical methods,P<0.05 considered that the difference was statistically significant.Results:1.Comparison of general information and biochemical indicators between the experimental group and the control group: The serum cystatin C and UACR levels in the experimental group were higher than those in the control group(P<0.05,the differences were statistically significant).And the age,triglyceride,homocysteine,lipoprotein(a)levels,and the proportions of hypertension,diabetes,and carotid arteriosclerosis in the experimental group were higher than those in the control group(P<0.05,the differences were statistically significant).The HDL-C was lower than that of the control group(P<0.05,the differences were statistically significant).There was no significant difference in gender,coronary heart disease history,total cholesterol,and LDL-C between the experimental group and the control group.2.Multivariate logistic regression analysis performed on the factors that were statistically different between the two groups: history of diabetes(P=0.008),HDL-C(P=0.012),Lp(a)(P=0.000),Cystatin C(P=0.008),UACR(P=0.000),and the differences were statistically significant(P<0.05).3.Pearson correlation analysis between serum cystatin C,UACR levels and NIHSS score in patients with cerebral infarction: Serum cystatin C level was positively correlated with NIHSS scores(r=0.363,P=0.000)in patients with acute cerebral infarction.UACR level was positively correlated with NIHSS scores(r=0.374,P=0.000)in patients with cerebral infarction(P<0.05,the differences were statistically significant).4.Correlation analysis between serum cystatin C,UACR and biochemical indicators:Pearson correlation analysis was performed on serum cystatin C,UACR and various biochemical indicators in patients with cerebral infarction.Serum cystatin C level was positively correlated with homocysteine(r=0.207,P=0.039),the difference was statistically significant(P<0.05).There was no significant correlation between serum cystatin C and other indicators.There was no significant correlation between UACR and various biochemical indicators.Conclusions 1.The increase of serum cystatin C and UACR may be independent risk factors of cerebral infarction.2.In patients with acute cerebral infarction,the higher the serum levels of cystatin C and UACR,the more severe the neurological impairment.3.Serum cystatin C and UACR can be used as predictors of the severity of neurological deficits in patients with acute cerebral infarction clinically.
