Objective: To analyze the short-term efficacy of neoadjuvant ra diochemotherapy on the treatment of low-to-moderate advanced recta l cancer and the effect of mmp-2 expression on cancer tissues.Rat e and the effect of mmp-2 expression in cancer tissues to summariz e clinical experience in order to better guide clinical treatment decis ionsMethods:Retrospective analysis of 119 cases of stage II / III moderate and low-grade advanced rectal cancer admitted to the First People’s Hospital of Changde from January 2018 to June 2019,including42 cases in the neoadjuvant chemoradiation reoperation group and 77 in the direct operation group example.Result: The maximum diameter of the tumor before and after neoadjuvant radiotherapy and chemotherapy decreased from 4.71 ±0.81 cm to 4.02 ± 0.68 cm,and the distance from the distal end of the tumor to the anal margin extended from 4.95 ± 1.34 cm to 5.55 ±1.21 cm,the difference was statistically significant(p<0.05);The total tumor downgrade rate before and after neoadjuvant chemoradiation was45.23%(19/42),the difference was statistically significant(p <0.05);The control rate of R0 was 95.23%(40 / 42)in neoadjuvant radiotherapy group and 81.82%(63 / 77)in direct operation group,respectively,the difference was statistically significant(p <0.05);The rates of anuspreservation were 85.71%(36/42)and 68.83%(53/77),respectively,the difference was statistically significant(p<0.05);The operative time of rectal cancer patients in the two groups was 243.50(185.75,311.25)min and 263.00(207.50,338.50)min,the intraoperative blood loss was 200(100,300)ml and 200(100,250)ml,respectively,and there was no statistical difference Significance(p> 0.05);The total postoperative complication rates were 19.05%(8/42)and 23.38%(18/77),respectively,the difference was not statistically significant(p> 0.05);Postoperative local recurrence rates were 2.38%(1/42)and 16.88%(13/77),respectively,and the difference was statistically significant(p<0.05).Postoperative cancer tissue MMP-2 positive expression rates were45.24%(19/42)and 72.73%(56/77),respectively,the difference was statistically significant(p <0.05);the recurrence rate of MMP-2 positive patients was higher than that of MMP-2 negative patients,the difference was statistically significant((p <0.05).Conclusion: Neoadjuvant radiotherapy and chemotherapy can reduce the size of the patient’s tumor,extend the distance from the distal end of the tumor to the anal margin,reduce the stage of the tumor,increase the rate of anus preservation,reduce the short-term local recurrence rate,inhibit the positive expression of MMP-2 in cancer tissue,and not increase the operation time Medium bleeding volume and risk of postoperative complications,The positive expression of MMP-2 in the patient’s cancer tissue is closely related to tumor lymph node metastasis and recurrence.
