Objectives: To observe the concentration level of serum C1q/TNF-Related protein-9(CTRP9)in patients with acute large artery atherosclerosis(ALAA)cerebral infarction,and investgate the clinical significance of serum CTRP9 concentration in patients with ALAA cerebral infarction.Methods:57 ALAA cerebral infarction patients who met the admission criteria and 41 non-cerebral infarction patients with extracranial and extracranial large artery atherosclerosis from the department of neurology of our hospital,30 patients who meet the inclusion criteria were randomly selected from the healthy subjects in the physical examination center of our hospital and included in the normal group,all patients were selected from January 2019 to January 2020 in Huaihua Hospital affiliated to University of South China,and the serum CTRP9 concentration level of the all patients was detected by enzyme linked immunosorbent assay(ELISA).According to the National Institutes of Health Scale(NIHSS),the ALAA cerebral infarction groupwas divided into three groups: mild group(1-4 points),moderate group(5-15 points)and severe group(≥16 points).All data were analyzed by SPSS25.0 software.Serum CTRP9 concentration levels among ALAA cerebral infarction group and non-cerebral infarction group,normal group and acute atherosclerotic cerebral infarction group with different degrees of neurological impairment were analyzed and compared.Results: 1.Compared with non-atherosclerotic group and healthy group,the concentration level of serum CTRP9 in ALAA cerebral infarction group was significantly lower than that in non-cerebral infarction large artery atherosclerosis group and healthy group(P<0.05),and the level of serum CTRP9 in large artery atherosclerosis group was significantly higher than that in healthy group(P<0.05);2.There was no significant difference in the cconcenteration level of serum CTRP9 among the three groups after pairwise comparison;3.Pearson correlation analysis showed that there was no significant correlation between the concentration level of CTRP9 and the NIHSS score of patients with ALAA cerebral infarction(r=0.092,P<0.05),and there was no significant difference in the concentration level of serum CTRP9 among the three groups(P>0.05),and there was no significant correlation between the concentration level of CTRP9 and the score of NIHSS in the group of ALAA cerebral infarction cerebral infarction(P>0.05);4.Logistic regression analysis showed that the OR value of CTRP9 was0.924(P <0.05),indicating that the concentration level of CTRP9 had a negative effect on the occurrence of cerebral infarction;5.The best cut-off point of CTRP9 for diagnosing ALAA cerebral infarction was200.28ng/ml,the sensitivity was 72%,the specificity was 83%,and the area under the receiver operating characteristic(ROC)was 0.826(95%CI,0.757-0.896,P < 0.001).The verification results show that the sensitivity of the model is 82%,the specificity is 85%,and the accuracy is 83%.The consistency coefficient of Kappa test is 0.658(P < 0.001).Conclusions: 1.The concentration level of serum CTRP9 was correlated with ALAA cerebral infarction and large artery atherosclerosis,and its concentration level was lower than that in non-cerebral infarction large artery atherosclerosis group;2.The concentration level of serum CTRP9 was not related to the severity of neurological impairment in patients with ALAA cerebral infarction;3.Serum CTRP9 may be one of the predictors of ALAA cerebral infarction and atherosclerosis.
