| Background:In recent years,epidemiological investigations have revealed that many patients have developed chronic viral B related cirrhosis.With the development of the disease,patients with cirrhosis may develop into liver cancer,which poses a great threat to the life and health of patients.Early diagnosis is the key to improve disease progression and reduce adverse prognosis in patients with post-hepatitis cirrhosis.At present,the liver hardness test is based on its new non-invasive imaging diagnostic equipment,which can obtain the liver hardness information of patients to a certain extent.In addition,the four liver fiber tests can also preliminarily determine the condition of liver fibrosis.Objective:To evaluate the diagnostic value of liver hardness test combined with liver fiber test.Methods:The basic information and clinical data(age,gender,liver hardness value and liver fiber value,and traditional ultrasound findings and diagnosis)of 105 patients with chronic post-hepatitis B cirrhosis hospitalized from July 2017 to March 2019 were retrospectively analyzed.The diagnostic efficiency of traditional ultrasound,single liver hardness test,single liver fiber test and liver hardness combined with liver fiber test were compared respectively.SPSS statistical software was used for comparative analysis.Results:(1)Liver hardness test:105 cases of hepatitis B cirrhosis patients with liver stiffness 3.4-75.OkPa,average LSM(51.53±19.4)kPa;(2)liver fiber four tests:hepatitis B cirrhosis patients HA(266.72±71.42 Ng/ml;LN(57.27±10.14)ng/ml;PCIII(75.94±17.03)ng/ml;CIV(75.94±17.03)ng/ml.(3)Traditional ultrasound diagnosis:105 patients with hepatitis B cirrhosis were diagnosed with 58 cases of cirrhosis by traditional ultrasound diagnosis,the detection rate was 55.24%;93 cases(88.57%)cirrhosis patients with liver section morphology was irregular;81 cases(77.14%)with portal vein dilatation;84 cases(80.00%)with right hepatic lobe expansion,78 cases(74.29%)with left hepatic lobe expansion;51 cases(48.57%)Ultrasound images showed that the liver capsule was not smooth;53 cases(50.48%)showed enhanced echogenicity and thickening of the liver parenchyma;49 cases(46.67%)showed that the echogenicity of the liver parenchyma was not uniform.(4)Comparison of diagnostic efficiency:the diagnostic efficiency of liver hardness test combined with liver fiber test was significantly higher than that of traditional ultrasound,single liver hardness test and single liver fiber test,which presented that the sensitivity,specificity,positive predictive value and negative predictive value were significantly different(P<0.05).Conclusion:Traditional ultrasound examination,liver hardness test and liver fiber test can provide certain diagnostic basis for the identification of hepatitis b cirrhosis.The detection efficiency of the three tests is different,but the overall diagnostic efficiency is not significantly different.Compared with the above three methods,the clinical efficacy of liver hardness test combined with liver fiber test in the diagnosis of post hepatitis cirrhosis and the evaluation of severity is better. |
