Objective To investigate the changes of blood lipid and its correlation with oxidative stress in Chronic Kidney Disease(CKD)patients,To provide evidence for lipid intervention and antioxidant therapy in CKD patients.Methods A total of 259 cases of CKD patients who were not treated with lipidlowering,dialysis or kidney transplantation from July 1,2017 to April 30,2019 were selected.Observed the patient’s gender,age and general indicators: the level of Hemoglobin(Hb),Albumin(ALB),Serum Creatinine(Scr),Cystatin C(CYS-C),Uric Acid(UA),Super Oxide Dismutase(SOD),C-Reactive Protein(CRP),observed blood lipid indicators: the level of Triglyceride(TG),Total Cholesterol(TC),Low Density Lipoprotein(LDL),High Density Lipoprotein(HDL).According to the blood lipid level,it was divided into the experimental group(abnormal blood lipid,135 cases)and the control group(normal blood lipid,124 cases).According to Glomerular Filtration Rate(GFR),the two groups of is divided into CKD1-2,3-4 and 5 period,compare the general indicators between two groups,and compare the general indicators and lipid indicators according to different CKD period in the experimental group.The experimental group was divided into 5 groups according to the characteristics of dyslipidemia: High in triglycerides(HTG)group,Low High Density lipoprotein(LHDL)group,HTG & LHDL group,Commixture Hyperlipidemia(CH)group and CH & LHDL group.Again compare the general indicators,the number of distributed cases in each group in different CKD stages was calculated.Finally,pearson correlation analysis was applied to define the correlation between indicators.SPSS 20.0 software was used for statistical analysis,P<0.05 for the difference was statistically significant.Results 1.The experimental group compared with the control group: There was no statistically significant difference in the levels of Hb,ALB,Scr,CYS-C,UA,SOD andCRP in CKD1-2 stage(P>0.05);There was no statistically significant difference in the levels of Hb,ALB,Scr,CYS-C,UA and SOD in CKD3-4 stage(P>0.05),but the CRP level of the experimental group was higher than that of the control group in CKD3-4stage(P<0.05);There was no statistically significant difference in the levels of Hb,ALB,Scr,CYS-C and UA in CKD5 stage(P>0.05),but the CRP level of the experimental group was higher than that of the control group,and the SOD level was lower than that of the control group in CKD5 stage(P<0.05).2.Compare in the experimental group: The levels of Hb,ALB and SOD gradually decreased with the GFR decline(P<0.05);The levels of Scr,CYS-C and UA gradually increased with the GFR decline(P<0.05);The CRP levels of CKD3-4 and 5 stage were significantly higher than those of CKD1-2 stage(P<0.05),there was no statistically significant difference in CRP level between CKD3-4 and 5 stage(P>0.05);There was no statistically significant difference between TG and TC levels with the GFR decline(P>0.05);The LDL level in CKD3-4 and CKD5 stage was significantly higher than that in CKD1-2 stage,and the HDL level was significantly lower than that in CKD1-2 stage(P<0.05),there was no significant difference in LDL and HDL levels between CKD3-4 stage and CKD5 stage(P>0.05).3.Compare in different blood lipid characteristics groups: The number of cases of HTG combined with LHDL blood level and simple LHDL blood level in CKD was significantly higher than the other 3 groups,and the number of cases increased significantly with the GFR decline.There was no statistically significant difference in ALB,UA and CRP levels between the 5 groups(P>0.05).Comparison of Hb level: LHDL group lower than CH group and HTG group(P<0.05).Comparison of Scr and CYS-C levels: LHDL group higher than CH group,HTG group and HTG combined with LHDL group(P<0.05).SOD level comparison: LHDL group lower than CH group(P<0.05).4.Correlation analysis: Hb level was positively correlated with TC,LDL and HDL levels(p<0.05).Scr level was negatively correlated with TG and HDL levels(p<0.05).The level of CYS-C was negatively correlated with the levels of TC,LDL and HDL(p<0.05).SOD level was positively correlated with HDL level(p<0.01).CRP level was negatively correlated with LDL level(p<0.05).Conclusion 1.The increase of CRP level and the decrease of SOD level in patients with abnormal blood lipid in CKD were more significant than those in patients with normal blood lipid.2.Increased CRP level and decreased SOD level were common in CKD,and the change was significant with the GFR decline.In patients with CKD dyslipidemia,with the GFR decline,there was no significant difference between TG and TC levels,LDL levels increased significantly and HDL levels decreased significantly.3.CKD with abnormal blood lipid was mainly characterized by HTG combined with LHDL blood level and simple LHDL blood level,and the number of patients increased significantly with the GFR decline.4.There was a correlation between lipid metabolism disorder and oxidative stress,and HDL level was most closely related to renal function.
