| Objective:Post-thrombotics syndrome of the lower extremity is a group of syndromes that have previously suffered from deep venous thrombosis of the lower extremity,most of which have been treated by informal clinical intervention,and have repeatedly appeared a series of clinical symptoms and signs.pts is a long-term chronic pathological process,with the passage of time,the symptoms of patients gradually aggravated,which not only increased the economic burden of patients,but also seriously reduced the quality of life of patients.along with the continuous development of intravascular interventional therapy,minimally invasive interventional therapy has gradually become the preferred method for lower extremity pts.aim of this study was to evaluate the clinical efficacy and experience of the pts of endovascular interventional therapy,to explore its optimal treatment timing and application principles,and to provide reference for clinical decision-making.Matterials and Methods:The clinical data of 63 patients with post-thrombotics syndrome(PTS)of the lower extremity treated in our hospital from 2016.1 to 2019.1were collected.According to the different treatment methods,the patients were divided into simple balloon dilatation group and balloon dilatation combined with stent implantation group.The clinical characteristics,treatment methods,symptoms and signs improvement before and after treatment were studied by retrospective study.The statistical analysis showed whether there were any statistical differences in scores,limb symptoms improvement and blood vessel patency rate during follow-up and the effect on the quality of life of the patients.Results:All patients receiving endovascular interventional therapy were successfully treated with surgery,and the success rate of immediate surgery was 100% withoutserious complications.All 63 patients underwent interventional endovascular treatment.Of these,28 patients underwent only balloon dilatation and 35 patients underwent iliac vein stent implantation after balloon dilatation.All the patients had improved clinical symptoms after operation,the quality of life was obviously improved than before,the pain,itching and swelling of the affected limbs were obviously alleviated after operation,the area of ulcer was gradually reduced,the healing trend was good,and the overall effective rate was 100%.follow-up time was6-12 months,at 6 months,all patients received follow-up(follow-up rate 100%);at 12 months,60 patients received follow-up(follow-up rate 95%).Three months after operation,the villalta scores of the two groups decreased from16.18±6.35、16.06±6.54 to 7.36±3.83、6.94±3.25;the CIVIQ-20 scores decreased from 61.11±8.61、58.97±8.43 to 36.61±8.46、35.77±6.58.the severity of the pts was significantly reduced and the quality of life was significantly improved.there was no significant difference in clinical symptoms and signs improvement(villalta score),quality of life improvement(civiq-20 score)and intravascular restenosis or secondary thrombosis between the balloon dilatation group and the balloon dilatation combined with stent implantation group(p>0.05).but at 12 months of follow-up,patients with balloon dilatation combined with stent placement were significantly better than patients with simple balloon dilatation in the above aspects,and the difference was statistically significant(p<0.05).Conclusions:Vascular endovascular interventional therapy PTS has a high technical success rate,which has a remarkable effect in improving the clinical symptoms of patients,reducing the severity of pts and improving the quality of life of patients,and has a good effect in the near and medium term.Balloon dilation combined with stent implantation has more significant intermediate effect than balloon dilation alone.but further studies are needed to determine the appropriate timing and optimal stent selection for stent placement.the long-term patency of the stent requires long-term follow-up for further observation. |
