Objective: 1.Observe the central lymph node staining and postoperative pathological results during the operation of differentiated thyroid cancer,and explore the application of nano-carbon tracing technology in the lymph node dissection of the central area of differentiated thyroid cancer;Whether type thyroid cancer surgery can reduce recurrent laryngeal nerve and parathyroid gland injury and speed up postoperative recovery;3.Summarize and summarize the practical application value of nano-carbon technology in clinical.Methods:Using retrospective analysis,65 patients who underwent thyroid cancer surgery in our general thyroid specialty from September 2017 to September 2019 were selected in our hospital.Postoperative pathological examination confirmed the differentiation of thyroid cancer.Preoperative radiographic examination showed no lymph node metastasis in the lateral neck area.Patients were divided into conventional surgery group(control group)and nano-carbon group(experimental group)according to whether or not nano-carbon was used during the operation.The operation was performed by the same clinically experienced thyroid surgery specialist.Surgical methods were performed on the affected thyroid gland lobe + Gorge + ipsilateral central lymph node dissection.The experimental group was injected with 0.2 ml of nano-carbon suspension according to the medication instructions after the glands were exposed during the operation.After successful staining,the surgical method was the same as the conventional group.Observe and record the number of lymph node dissections in the central area of the experimental group and the control group,and analyze the postoperative incision drainage,bleeding during surgery,operation time,parathyroid injury cases,hospitalization days,postoperative hoarseness cases,postoperative blood Calcium temporarily decreased the number of cases.Results:1.General data: 65 patients were selected from the above-mentioned patientswho met the experimental criteria,including 33 in the nano-carbon group(experimental group)and 32 in the conventional surgery group(control group).There were 12 males and 21 females in the experimental group,with an average age of 48.33 ± 6.78 years.There were 10 males and 22 females in the control group,with an average age of 47.31± 6.18 years.Comparing the preoperative age and sex of the two groups of patients,No significant difference between the two groups of data(P> 0.05),indicating that the two groups are comparable.2.The results showed that the average operation time in the control group was94.63 ± 11.38 minutes,the average operation time in the nano-carbon group was91.73 ± 8.69 minutes,the average postoperative hospital stay in the control group was7.53 ± 1.88 days,and the average length of hospital stay after surgery in the nano-carbon group was 6.70 ± 1.67 Days: The incision drainage after surgery in the control group was 27.03 ± 6.23 ml,the nano-carbon group was 26.15 ± 6.42 ml,Bleeding volume during surgery in the control group was 28.06 ± 6.65 ml,and the nano-carbon group was 26.27 ± 7.36 ml.There was no significant difference between the two groups(P> 0.05).3.The number of parathyroid glands removed by mistake and the number of central lymph nodes by the conventional surgical group and the nano-carbon group were compared.Among them,6 cases in the control group were misdiagnosed,accounting for18.75%;1 case in the nano-carbon group was cut by mistake,accounting for 3.03%.the mistake rate in the control group was significantly higher than that in the control group,and there was a significant difference between the two groups(P <0.05).The number of central lymph node dissection in the control group was 5.59 ± 1.86,and that of the nano-carbon group was 8.24 ± 1.54.and the differences between the two groups were statistically significant(P <0.05);4.There were 2 cases of hoarseness after operation in the nano-carbon group(6.06%),2 cases of temporary decrease in blood calcium(6.06%),and 6 cases(18.15%)and 7 cases(21.88%)of hoarseness in the control group.There was no significant difference between the two groups(P> 0.05).Conclusion:1.The application of nano-carbon tracing technology in the operation of differentiated thyroid cancer has no effect on the operation time,intraoperative blood loss,postoperative incision drainage volume and length of hospital stay,and the application of this technology can not accelerate the recovery rate of patients.2.The application of nano-carbon tracing technology in the operation of differentiated thyroid cancer can significantly increase the number of lymph nodes in the central area,thereby reducing the risk of tumor recurrence.3.The application of nano-carbon tracing technology in the operation of differentiated thyroid cancer can effectively identify and protect the recurrent laryngeal nerve and parathyroid gland during surgery,reduce the damage of recurrent laryngeal nerve and protect the parathyroid gland.
