| OBJECTIVE To study the microstructure of the relevant bone tissue in patients with femoral head necrosis or knee osteoarthritis,and to analyze the related structural changes based on the results obtained.METHODS The femoral head specimens of patients with severe femoral head necrosis undergoing hip arthroplasty were collected.The tibial plateau specimens of patients with knee osteoarthritis undergoing single ankle replacement and total knee replacement were collected.Micro-CT scans were performed.Dataviewer and CTAn software were used to analyze three-dimensional microstructure of bone tissue.RESULTS A total of 47 cases were collected in this study,including 6 cases of knee joint control group,8 cases of single iliac replacement,21 cases of total knee replacement,and 12 cases of hip arthroplasty.In the comparison of STB and DTB in the femoral head,paired t-test showed statistical differences between BV/TV,Tb.N,Tb.Sp,Tb.Th,DA and Conn.D.In the medial tibial plateau,one-way ANOVA showed statistically significant differences in BV/TV,Tb.N,Tb.Sp,Tb.Th and Conn.D.DA was not statistically significant.In the lateral tibia platform,one-way analysis of variance showed statistically significant differences in Tb.N,Tb.Sp,Tb.Th,and Conn.D between the control group and the single-replacement group,and the control group and the total knee replacement group.BV/TV,DA was not statistically significant.CONCLUSIONS There is a significant change in the microstructure of the subchondral bone in patients with different types of osteoarthritis.A comparative analysis of the subchondral cancellous bone and deep cancellous bone of the femoral head revealed significant differences in the microstructure of the two.By comparing the normal tibial plateau,single iliac replacement and total knee replacement,the microstructure of the three subchondral bones was significantly different.During the development of osteoarthritis,the subchondral bone was affected to varying degrees,showing a microstructural change different from that of the normal control group.The microstructural features of the subchondral bone were mainly characterized by increased bone density,increased bone mass,thickened subchondral bone plate,increased trabecular thickness,trabecular bone separation and reduced bone marrow spacing.In severe patients,the subchondral cancellous bone is different from the deep cancellous bone due to the destruction of the subchondral bone plate.In the osteoarthritis patients,the microstructure of the subchondral bone gradually deteriorates and the strength decreases.These bone changes are closely related to the development of diseases in patients with osteoarthritis. |
