Challenges And Countermeasures In The Development Of Medical Alliance

Developing the Medical Alliance is an innovating action to deepen the reform of the medical system,which could solve the problems on insufficient,uneven distribution,unequal and unreasonable allocation of medical resources.The outline of the Mational Medical and Health Service system Planning(2015-2020),issued by the General Office of the State Council in March 2015,put forward a clear request for the development of the medical allianceand affirmed its role in promoting the hierarchical diagnosis and treatment system.With the view of health justice,the experience of Zhangjiagang Aoyang Hospital which is the member of Shanghai Tenth People’s Hospital Medical Alliance was analyzed with investigated data to explore how to improve the development of medical alliance and play more important role in practice.Through data analysis,literature research and personal interviews to achieve the current status of Aoyang Hospital in the Medical Alliance construction process,we found the difficulties were as following,the level of recognition to the medical alliance was not high from patients,communities and medical staff,the process of hierarchical diagnosis and treatment and two-way referral were not smooth,the operating mechanism of medical alliance was not perfect,and there was lack of an interconnected information platform.With further analysis,we discussed some corresponding causes,such as lack of understanding to the social sources participated in the providing medical services and to the deep functions of the medical alliance,some subjective and objective factors from the patient,less of motivation to internal integrate,and some interest conflict within the stakeholders.Then some countermeasures and suggestions was developed to improve the medical alliance,including to strengthen the self-discipline in the process of providing medical services by social sources,to incorporate preventive health care into social grid governance,to change the promotional mode,to improve the operational mechanism,and to implement the linkage reforms with medical,insurance and medicine.Through the research and exploration,under the condition that the original medical resources have not increased significantly,the medical alliance should take more measures at the same time to optimize the mechanism,and the key point would be based on the principles of"equality and reciprocity,resources sharing,win-win cooperation,voluntary autonomy".Those measures could be taken based on the features of the medical alliance including that innovating the vertical integration model,optimizing the mechanism of division of labor cooperation,updating the mode of integrated mediocrity and emergency,improving the system of staff allocation and incentive,and establishing the structure to share resources and benefits.
