| BackgroundThe high incidence of stress urinary incontinence severely affects the quality of life and physical and mental health of women.Pelvic floor muscle training is the preferred method for the prevention and treatment of mild and moderate urinary incontinence,but patients have poor adherence to long-term maintenance of pelvic floor muscle exercise and irregular movements,which seriously affects the effect of exercise,and clinically evaluates the effectiveness of PFMT has various time,space,and health constraints。Objectives1.Adopt qualitative research methods,in-depth discussion of the analysis of maternal needs for a non-invasive and portable pelvic floor muscle training system,providing a theoretical basis for subsequent instrument construction2.On the basis of fully searching the retrieved literature,based on market research and demand analysis,build a model of a new type of pelvic floor muscle training system,explore the best surface electromyographic position for different positions and stability and safety of the system3.Combined with the pelvic floor muscle biofeedback device commonly used in clinic,the experimental effect evaluation of the new device in the treatment of stress incontinence was discussed.Methods1.Demand analysis:Take phenomenological methods in qualitative research as guidance,23 patients with stress urinary incontinence who were treated in a postpartum pelvic floor rehabilitation clinic of a third-class hospital in Shenzhen for pelvic floor rehabilitation equipment were selected for semi-structured interview.The Colaizzi analysis method was used to analyze the interview data and determine the key themes.2.Model building:Through fully searching the retrieved literature and needs analysis,construct a new exercise device model,select 20 volunteers with normal pelvic floor muscle function,and determine the best surface EMG collection site in different positions.3.Feasibility analysis:The experimental subjects were treated with three methods:conventional pelvic floor muscle exercise method,clinical commonly used biofeedback instrument method,and new instrument method.One month later,the ICIQ-SF-Chinese scores of the three groups of subjects and the pelvic floor muscle strength before and after treatment were compared;a qualitative research method was used to explore the user’s experience with the two devicesResults1.Demand analysis of training equipment can be divided into 3 categories and 6 topics:existing instruments have many deficiencies;new equipment needs to expand remote rehabilitation training to relieve medical pressure;new equipment needs non-invasive auxiliary training;new equipment need to be economical;new instruments need to solve the deficiencies of conventional instruments.2.According to the previous demand analysis,build a new model of pelvic floor muscle function exerciser.When standing,the electrode pads on the upper and lower sides of the coccyx muscle are the best positions to collect surface EMG signals;in the supine and sitting positions,the left and right sides of the perineum are the best positions.The system measurement is stable(P>0.05)and has good safety performance.3.Before and after the experiment,there was no significant statistical difference between the efficacy of the new instrument and the conventional instrument(P>0.05),but both were better than the blank control group,but the user’s evaluation of the comfort and portability of the new instrument was higher than that of the conventional instrument(P<0.05).Conclusion1.The needs of the new equipment include small and portable appearance,non-invasive training,online rehabilitation,and economic benefits.The existing pelvic floor muscle function evaluation and training equipment have many deficiencies that need to be improved.The market is broad and the acceptance is high.2.Non-invasive,non-position restricted pelvic floor muscle training system combines integrated circuit technology,surface electromyography technology,and pelvic floor muscle exercise to comprehensively treat stress urinary incontinence,which can effectively solve the shortage of inherent instruments and optimize the allocation of medical resources.3.The feasibility analysis of the new instrument is good,and it has many advantages compared with the conventional instrument.It also provides ideas for the later optimization of the instrument. |
