Anatomical Observation And Histological Study Of Denonvilliers Fascia

Objective: To study the histological source of Denonvilliers fascia through the anatomy and observation of the corpse Denonvilliers fascia,the pathological detection of the specimen,and the observation of the surgical specimens.Protection provides a theoretical basis and provides more theoretical support for clinical surgery.Methods: 40 adult corpses(20 males and 20 females)fully fixed with formaldehyde were dissected.The anatomy of the whole pelvic specimen was performed according to the requirements of laparoscopic TME surgery,and the half pelvic specimen was dissected according to the requirements of local anatomy,the Denonvilliers fascia and its adjacent tissues were fully exposed,and the anatomical features of Denonvilliers fascia were observed.Denonvilliers fascia tissue was removed from the cadaver specimens,sliced,stained and observed under a microscope.Denonvilliers fascia tissue composition;selected Denonvilliers fascia intact,fascia microvascular or microvascular network clearly visible 5 patients with rectal cancer Observe.Results: It was observed that most Denonvilliers fascias have the traditionally considered classic position structure: the head side starts from the rectum and bladder depression(or rectum and uterine depression)peritoneal reflex,runs vertically to the caudal side and stops at the perineum,towards The two sides are fused with the urogenital fascia.In the pelvic wall,there are pelvic nerve plexus and urogenital neurovascular bundle between the deep and shallow urogenital fascia.Denonvilliers fascia is adjacent to the rectum and seminal vesicles,prostate(male)or uterus,vagina(female).Denonvilliers fascia is a layer of tough fascia-like tissue with a general appearance of "tongue".Of the 40 corpses,one male specimen did not see Denonvilliers fascia,and one male specimen Denonvilliers fascia formed vacuoles below the peritoneal reflex,and Denonvilliers fascia showed a two-layer structure locally.The peritoneum continues with Denonvilliers fascia and constitutes a typical "Y"-like structure.From the gross specimens(cadavers and surgical specimens),it can be clearly observed that there are tiny blood vessels or a network of tiny blood vessels extending from the peritoneum to the Denonvilliers fascia.Specimens obtained from cadavers(samples with peritoneum and Denonvilliers fascia in the shape of "Y")were observed under light microscope: the peritoneum and Denonvilliers fascia have the same tissue composition,and the basal layer of their cells is continuous.Conclusion:(1)The Denonvilliers fascia is a layer of tough and tough fascia-like tissue with a general appearance of "tongue".The Denonvilliers fascia is present in most individuals and has the traditional position relationship of classics..(2)Denonvilliers fascia is a fusion fascia formed by the fusion of the peritoneum.It has homology with the peritoneum in histology.It plays an important role in preventing tumor metastasis.(3)In front of the rectum,the superficial Denonvilliers fascia and the urogenital fascia are tightly fused at the bottom of the prostate and are not easy to separate;on the side of the rectum,the Denonvilliers fascia and the urogenital fascia fascia are tightly fused and difficult to separate.There are pelvic nerve plexus and urogenital neurovascular bundle between the deep and superficial layers of urogenital fascia,and the relationship between them is very close.(4).Total mesenteric resection of rectal cancer(TME)The surgical level of the anterior rectal space requires different surgical planes according to the size,stage and location of the tumor.
