Research Purpose:To investigate the medication compliance level of schizophrenic patients in yangzhou city.To explore the relevant influencing factors of medication compliance of schizophrenic patients under the guidance of the health belief theory model,so as to provide reference basis and research ideas for the later clinical empirical studies to improve medication compliance,so as to improve the recovery of patients.Research Methods:This study conducted a cross-sectional survey of schizophrenia patients in urban areas of yangzhou city from May 2018 to December 2019 to conduct household or community health service center investigation,and 256 patients were conveniently selected as research subjects.The survey tools included the general questionnaire,the Morisky medication compliance questionnaire(MMAS-8),the medication attitude inventory(DAI),the self-awareness rating scale(SAI),the stigma rating scale(SSMI-C),the social support rating scale(SSAS),the side effects(TESS),and the brief psychiatric scale(BPRS).In addition,according to the health belief model,we used the intermediary model to analyze the mediating effect of medication attitude between mental symptoms,drug side effects,insight,shame,social support and medication compliance,using the PROCESS 3.3Macro Program compiled by Hayes and SPSS 23.0 to test the intermediary model.Research Results:1.In this study,patients with medication adherence level is(6.00 ± 1.48)points,medication adherence rate(full compliance + partial compliance)is 82.5%,the patient’s general condition of the single factor analysis results showed that the patients compliance levels in patients with guardian on significant differences(P < 0.05),with their parents,spouses,as guardian has high drug compliance.There were significant differences in medication attitude(6.06±1.19),psychiatric symptoms(35.27±9.88),drug side effects(1.79±0.96),social support(57.67±12.09)and stigma(52.34±15.26)between the adherence group and the non-adherence group(P < 0.01),while there were no differences in self-knowledge(18.21±4.44)(P > 0.05).2.Using the single factor linear regression analysis found that attitude(β= 0.843,P < 0.001),disease shame(β = 0.067,P < 0.001),social support(β = 0.082,P < 0.001),mental symptoms(β = 0.083,P < 0.001),side effects(β = 0.732,P < 0.001)and self-knowledge(β = 0.067,P < 0.001)can predict medication adherence.3.Multiple stratified regression analysis was used to control confounding factors to explore the effects of various factors in medication compliance model on drug compliance in patients with schizophrenia.The results of the first layer model show that demographic data can not well predict drug compliance(P﹥0.05).The second layer model showed that psychiatric symptoms(β =-0.05,P <0.001)and drug side effects(β =-0.535,P < 0.001)could predict drug compliance.In the third layer,the sense of shame(β = 0.015,P < 0.001),social support(β = 0.018,P < 0.001)and medication attitude(β = 0.485,P<0.001)could predict drug compliance,and their R2 was 0.897,an increase of 0.459,indicating that these variables could explain 45.9% of the total variation of patients’ drug compliance.4.The intermediary model shows that medication attitude completely mediates the relationship between mental symptoms,insight and medication compliance.In addition,drug attitude partially mediates the relationship between drug side effects,shame,social support and drug compliance.Research Conclusion:1.The low level of medication compliance in schizophrenia patients in this study indicates that patients living at home have an increased risk of disease recurrence or re-hospitalization,thus increasing the burden on the family and society,and improving medication compliance is crucial.2.In this study,the self-awareness of guardians of patients with schizophrenia,stigma,social support,psychiatric symptoms,medication attitude,medication side effects will affect patients’ medication compliance level,suggesting that we can provide targeted intervention measures.3 In this study,schizophrenia patients with psychotic symptoms,drug side effects,social support,self-knowledge,stigma can influence patients’ medication compliance through the mediating effect of medication attitude,which provides reference basis and scientific research ideas for the later research on the corresponding intervention measures in this population...
