Research Purpose:The purpose of this study is to introduce the Gatos Clinical Test(GCT)Questionnaire from Greek for patients with severe dementia,to translate,back-translate and adjust for cultural adaptation.The Chinese version of GCT questionnaire was revised according to the background of Chinese medical culture.The reliability and validity of the Chinese version of GCT questionnaire were evaluated to explore its applicability in the population with severe dementia in China through objective quantitative indicators.Research Methods:1.With the consent and authorization of the original questionnaire developer,a formal questionnaire will be formed after systematic translation,back translation,cultural adjustment and preliminary investigation.2.Patients with severe dementia in Huzhou Third Municipal Hospital from January 2019 to August 2019 were selected as the survey object.The survey data was entered after double-checked and analyzed by SPSS23.0 statistical software.We used item selection and factor analysis for the formal questionnaire.The validity of the questionnaire was evaluated by content validity and structure validity,and the reliability of the questionnaire was tested by internal consistency reliability,test-retest reliability and inter-observer reliability.Research Results:1.The Chinese version of GCT questionnaire consists of two parts.First,it includes 6general information related to the patient's pathological characteristics(A:hearing,B:eyesight,C:general status,D:patient behavior,E:facial expressions,F:walking-movement).The main part is composed of 3 dimensions and 14 items.The three dimensions are autonomy/alertness(5 items),agnosia(5 items)and somatokinetic function/ sleep(4 items).The expressions of 14 items are easy to understand,concise and clear,which conform to the medical cultural background of our country.2.The validity of GCT questionnaire:The content validity index(CVI)of the total questionnaire was 0.91,and the content validity index of each item ranged from 0.80 to 1.00.Three common factors were extracted through exploratory factor analysis,and the characteristic valuesof each factor were 6.300,1.704 and 1.167 respectively.The cumulative variance contribution rate was 65.513%.The factor loading coefficient of each item was between 0.519 and 0.902.The correlation coefficient between each item and the total questionnaire was 0.487 ~ 0.895.The correlation coefficient between each item and the attribution factor was between 0.598 and0.921.The correlation coefficients between the three dimensions and the whole questionnaire were0.946,0.766,and 0.736 respectively.Correlation coefficients between the three dimensions were0.591,0.600,and 0.387 respectively.The correlation of questionnaires was significant.3.The reliability of GCT questionnaire:Cronbach's a coefficient of the total questionnaire was 0.898,Cronbach's a coefficients of each dimension were 0.901,0.734 and 0.850.The inter-observer reliability of the total questionnaire was 0.986,the inter-observer reliability of three dimensions were 0.976,0.975 and 0.877 respectively.The test-retest reliability of the total questionnaire was 0.959,the test-retest reliability of three dimensions were 0.951,0.953 and 0.882 respectively.Research Conclusion:The Chinese version of GCT questionnaire for patients with severe dementia has good reliability and validity.The dimensions and items of the questionnaire can systematically and comprehensively assess patients' autonomy,cognitive ability and physical function.The number of items and evaluation time are moderate.It can be used as a tool for clinical and community health care professionals to assess patients with severe dementia.
