Objective To investigate the distribution of high-order aberrations on the anterior corneal surface of patients with age-related cataract, and to explore the correlation between factors such as axial length, corneal curvature, age, corneal Q value and high-order aberrations on the anterior corneal surface. The selection of spherical IOL provides a reference. Methods Prospective research design. From June to August 2018,138 patients(138 eyes)underwent cataract surgery in the Department of Ophthalmology, the First Affiliated Hospital of Anhui Medical University, including 65 males(47.1%)and 73 females(52.9%),aged 50 to 93(68.5 ± 10.6)years old.IOL Master was used to measure the axial length of 138 eyes of 138 patients.Allegro Oculyzer anterior segment analyzer was used to measure the high-order aberrations of the corneal anterior surface, corneal curvature, corneal Q value, corneal astigmatism, and the central corneal 6mm anterior corneal surface Root mean square value of coma, spherical aberration and clover aberration, using spearman correlation analysis to analyze the correlation with eye axis length, corneal curvature, age, corneal Q value and other factors;Aberrations with clover were dependent variables, and age, corneal Q value, corneal astigmatism, corneal curvature, and eye axis length were used as independent variables for multiple linear regression analysis;patients were divided into long eye axis group and middle eye axis group according to eye axis length 3,the short eye axis group, according to the corneal curvature, the patients were divided into low curvature group and high curvature group, and compare the difference of high-order aberration of corneal anterior surface between the groups. Results Anterior corneal coma, spherical aberration, and axial length were negatively correlated(r =-0.192,-0.225;P <0.05);There was no correlation between the clover aberration on the anterior surface of the cornea and the axial length;Spherical anterior surface of the cornea. There was a positive correlation with the average corneal curvature(r = 0.173;P <0.05);There was no correlation between the average corneal curvature and the anterior corneal coma and clover aberration; The age was related to the anterior corneal surface coma, spherical aberration, and clover image The differences were positively correlated(r = 0.324,0.247,0.350;P <0.05);The anterior corneal spherical aberration was significantly higher in patients with low curvature in the short axis group than in the middle and long axis groups, and there were differences between the three groups.Statistical significance(χ~2 = 8.404,P = 0.015);There was no significant difference between the high-order aberrations of the anterior surface of the cornea in the low curvature group and the high curvature group. Conclusion The high-order aberrations of the anterior corneal surface of age-related cataract patients are related to a variety of factors, and the situation of the patient’s eye needs to be comprehensively considered before surgery.According to the corneal optical characteristics, age, axial length, corneal curvature, corneal Q value, etc. And personalized selection of intraocular lens.
