Objective:The object of this article is to explore the risk factors related to Early-onset type 2 diabetes mellitus(EOT2DM)and the relationship with Selenium(Se)level,and the effect of selenium supplementation on insulin resistance(IR)and hypersensitive C-reactive protein(Hs-CRP)in Early-onset type 2 diabetes mellitus with low selenium.Methods:1.128 patients in outpatient departments of endocrine clinic or hospitalized type 2 diabetes mellitus in our hospital were selected as the study group from January 2019 to December 2019,of which 72 patients with the EOT2 DM were in group A,and 56 patients with th e LOT2 DM were in group B,as well as 52 person with healthy peo ple in the physical examination center were the control group C in t he same period,the total number of people mentioned above is 180 cases.42 patients with blood selenium lower than 100.15ug/L in gro up A were divided into A1 group and A2 group with 21 cases in ea ch group respectively,of which group A1 was given selenium-supple mented yeast tablets 100ug/d for 12 weeks.2.The biochemical indexes of Serum Se,Hs-CRP,blood glucose,blood lipid,liver and kidney function in each group were measured,HOMA-IR was calculated,and the differences of indexes were compared in each group;the correlations between Serum Se,Hs-CRP and HOMA-IR,blood glucose,blood lipid,liver and kidney function were analyzed.3.Using serum selenium as the test variable,and EOT2 DM and LOT2 DM as the state variables,the ROC curve of draw serum seleniumwas calculated to find the predictive value of serum selenium incidence risk for EOT2 DM and LOT2 DM.4.Changes in biochemical indicators such as serum selenim,Hs-CR P,HOMA-IR,blood glucose,blood lipid,liver and kidney function were compared before and after treatment in the A1 and A2 groups.Result:1.Compared with group C: the serum selenium level of group A and group B decreased,the level of Hs-CRP increased,the positiv e Diabetes with family history and the incidence of overweight or ob esity increased,the difference was statistically significant(P<0.05).2.Compared with group B: the serum selenium level of group A decreased,the level of Hs-CRP increased,the positive rate of Dia betes mellitus with family history and the incidence of overweight or obesity increased,the difference was statistically significant(P<0.05).3.Correlation analysis: The Serum selenium was negatively corre lated with Hs-CRP and HOMA-IR(P<0.05);Hs-CRP was positively c orrelated with HOMA-IR(P<0.05).4.Using serum selenium as the test variable,EOT2 DM and LOT2 DM as the state variables,the AUC calculated by the ROC curve of draw serum selenium was 0.907(P<0.05)and 0.657(P<0.05),respectively.Serum selenium has a more predictive value for the incidence risk of EOT2 DM.5.The Hs-CRP,HOMA-IR,FPG,FINS,Hb A1 c,TG,TC,and LDL-C of after treatment in A1 and A2 groups were lower than those before treatment(P<0.05),and the reduction range was A1> A2(P<0.05).Conclusion:1.The blood selenium level of patients with type 2 diabetes mellitus is lower than that of healthy people,among them,the patients with early-onset type 2 diabetes mellitus have lower blood selenium levels.2.Low selenium can be used as a predictor to predict the risk of early onset type 2 diabetes mellitus.3.Selenium supplementation can improve oxidative stress,reduce insulin resistance and improve glucose and lipid metabolism disorder4.The patients with early onset type 2 diabetes mellitus have more obvious familial tendencies and higher incidence of overweight or obesity than those with late onset type 2 diabetes mellitus.
