| Objective:In order to provide a theoretical reference for clinical treatment,improve the diagnosis and treatment level of basal ganglia cerebral hemorrhage,and promote the healthy recovery of patients,this paper aims to explore the evaluation of curative effect and prognosis of hypertensive basal ganglia cerebral hemorrhage(20-40ml)based on diffusion tensor imaging(DTI)Methods:Seventy-four patients with hypertensive basal ganglia intracerebral hemorrhage who were treated in our hospital from January 2017 to January2019 were selected as the study objects.According to different treatment methods,they were divided into medical treatment group(22 cases),improved precision puncture group(28 cases)and small bone window minimally invasive surgery group(24 cases).The medical treatment group received DTI and Glasgow coma scale(GCS)scores within 24 hours after admission and two weeks after treatment,and the two groups received DTI examination and GCS scores within 24 hours before surgery and two weeks after surgery.Glasgow outcome(GOS)score of each group were measured after 3 months.The difference of FA value between the three groups before and 2 weeks after treatment was compared.The FA value of DTI in each group after two weeks and GOS score after three months were analyzed.Spss21.0 statistical software package is used for data analysis.The measurement data were indicated by(sx ?),the multiple groups were compared by analysis of variance(F test).The Spearman correlation analysiswas performed,P<0.05,indicating that the difference was statistically significant.Results:The difference of FA values between 24 hours before operation and 2weeks after operation in the improved precise puncture group was smaller than that in the small bone window minimally invasive operation group and the group of internal medicine treatment,the difference was statistically significant(P<0.05).The FA value of DTI after two weeks was positively correlated with the GOS score after three months(r = 0.754,P<0.01).Conclusion:DTI was used to evaluate the efficacy of three treatment methods for basal ganglia intracerebral hemorrhage.Improved precision puncture is more conducive to the recovery of corticospinal tract injury and improve long-term prognosis,which is worthy of extensive clinical promotion and application. |
