| Objective This study takes colonoscopy and pathological examination as the "gold standard",and compares the results of fitdna combined with colonoscopy and pathological examination to evaluate the clinical application value of this new detection technology in predicting colorectal cancer(CRC)and advanced adenoma.Methods The stool samples of 126 patients confirmed by colonoscopy and pathological biopsy were collected retrospectively,including 20 cases of CRC,15 cases of advanced adenoma,9 cases of non advanced adenoma,35 cases of non neoplastic polyp,and 47 cases of normal colonoscopy as control.The feces of 126 patients were detec ted by multi-target fecal fit-dna,and the results were statistically an alyzed.Multiple target fecal fit-dna combined detection extracts the DNA of intestinal exfoliated cells after pretreatment of collected fecal samples.Abnormal cells lead to the increase of BMP3,NDRG4 gene methylation and K-ras gene mutation level in fecal DNA.The sechanges are significantly related to CRC.Through qualitative det ection,126 fecal samples were collected with BMP3 and NDRG4 g ene methylation,K-ras gene mutation and fecal samples For the amount of hemoglobin,the information detected is used to generate a specific value using a logistic regression algorithm.By comparing with the normal population,we determined the threshold to be 165.If the value is lower than 165,the test result is negative;if it is greater than or equal to 165,the test result is positive.Results In this study,126 samples were included and divided into 5 groups: CRC group,advanced adenoma group,non advanced adenoma group,non neoplastic polyp group and normal endoscopy group.Among them,20 cases were included in CRC group,15 in advanced adenoma group,9 in non advanced adenoma group,35 in non neoplastic polyp group and 47 in normal group.Among the 126 cases,78(61.9%)were male and 48(38.1%)were female.The average age was 61 ± 3.5 years.The positive rate of multi-target fecal fit-dna was 100%(20 / 20),the sensitivity was 100%;the positive rate of advanced adenoma was 53.3%(8 / 15),the sensitivity was 53.3%;the positive rate of non advanced adenoma was 0%(0 / 9),the specificity was 100%;the positive rate of non neoplastic polyp was 5.7%(2 / 35),the specificity was 94.2%;the positive rate of enteroscopy was normal The positive rate was 12.7%(6 / 47),and the specificity was 87.2%.Based on the above results,126 subjects can bedivided into two groups,including CRC and advanced adenoma in 35 cases in one group,and the remaining 91 cases in the other group.According to ROC curve,the sensitivity of multi-target fecal fit-dna combined detection for CRC and advanced adenoma was 77.8%,the specificity was 92.2%,and AUC was 0.850.Conclusion Multi-target fecal FIT-DNA detection is highly sensitive to CRC and advanced adenomas,which can improve the accuracy of CRC and advanced adenoma screening,and is specific for non-advanced adenomas,non-tumor polyps,and normal populations.High sex can exclude non-cancerous diseases of the colorectum.Multi-target fecal FIT-DNA combined detection technology is suitable for screening of CRC and advanced adenomas. |
