| ObjectiveBy measuring male alcoholic fatty liver disease(alcoholic fatty liver diseases,AFLD)and nonalcoholic fatty liver disease(nonalcoholic fatty liver diseases,NAFLD)in patients with intestinal bacteria metabolites D-lactic acid(D-LA),intestinal mucosa product diamine oxidase(DAO)and the body’s inflammatory status indicators,Correlation analysis was made on liver function enzyme, blood lipid and intestinal permeability index, blood lipid, serum diamine oxidase and inflammatory index.,to investigate the changes of intestinal mechanical barrier function in male patients with fatty liver disease(FLD).MethodsA total of 213 males who underwent physical examination in the physical examination center of the first affiliated hospital of anhui medical university from march to August 2019 were selected, the 91 male NAFLD patients who met the diagnostic criteria for NAFLD were in the NAFLD group,40 male AFLD patients who met the diagnostic criteria for alcoholic fatty liver disease were in the AFLD group and a total of 82 males with normal physical examination during the same period were taken as the normal control group.Questionnaire survey, physical measurement, liver ultrasonic examination and blood examination were conducted on the candidate population.Collect the physical examination report, use the self-designed fatty liver questionnaire to investigate the general condition of the individual, and collect peripheral venous blood.Alanine transaminase(ALT),aspartate aminotransferase(AST),total cholesterol(TC),triglycerides(TG),high density lipoprotein cholesterol(HDL-C),very low density lipoprotein cholesterol(VLDL-C)andProtein(CRP)were measured with an automatic biochemical analyzer.Enzyme-Linked Immunosorbent Assay(ELISA)was used to determine serum levels of DAO,D-LA,TNF-α,IL-1 and IL-6.The changes of intestinal permeability, intestinal barrier function and inflammatory index were compared and the correlation was analyzed.ResultsIn this study, there was no significant difference in age among the three group patients(H=3.442,P=0.179).BMI,ALT,AST,TG,VLDL-C and diamine oxidase(DAO)were higher in NAFLD group and AFLD group than in control group,HDL-C in NAFLD group and AFLD group was lower than that in control group,D-LA in NAFLD group was higher than that in control group,TC in AFLD group was higher than that in control group, the difference was statistically significant(P<0.05).Inflammatory indicators indicated that the serum levels of TNF-α,IL-6 and CRP in the NAFLD group and AFLD group were higher than those in the control group, and the concentrations of TNF-αand CRP in the AFLD group were higher than those in the NAFLD group, with statistically significant(P<0.05).Meanwhile,correlation analysis showed that DAO was positively correlated with ALT,AST,TC,TG and VLDL-C(r_s>0,P<0.05),DAO and D-LA were negatively correlated with HDL-C(r_s<0,P<0.05).In NAFLD group and AFLD group, serum IL-1βand IL-6 were positively correlated with TG and VLDL-C(r_s>0,P<0.05),and serum IL-1βwas positively correlated with DAO(r_s>0,P<0.05).ConclusionThe serum level of DAO and D-LA in NAFLD patients were significantly increased, while the serum level of DAO also increased in AFLD patients, suggesting that intestinal mechanical barrier function in patients with fatty liver disease was impaired compared with normal population. The increased serum level of TNF-α,IL-6 and CRP in patients with NAFLD and AFLD,and the level of TNF-αand CRP in AFLD patients were higher than NAFLD patients, suggesting that the inflammatory level in fatty liver disease patients was higher, and the inflammatory level in AFLD patients might be higher.According to the correlation analysis results, the higher the degree of liver damage and the higher the level of lipid in patients with fatty liver disease, the higher the intestinal permeability will be,and the higher the level of lipid and intestinal permeability, the higher the level of inflammation in the body will be. |
