Purpose:A retrospective study of MRI images was used to analyze whether the degree of fat infiltration of the paraspinal muscles(multifidus,erector spinae,and psoas major muscles)was a risk factor for degeneration of the facet joints of the lumbar spine.Method:This subject reviewed the MRI image data of 2860 cases who were limited to 20-50 years old and received MRI examination of the lumbar spine at the Medical Examination Center of Hunan Provincial People’s Hospital from June 2017 to November 2019.Hunan Province People’s Hospital PACS system collected cross-sectional T2 MRI images at the level of L4,L5,and S1 vertebral body endplates to study the paravertebral muscles of the L3 / 4,L4 / 5,and L5 / S1 segments(multicleft Muscle,erector spinae,psoas major muscle)fat infiltration degree and facet joint morphology.A total of 216 cases met the case inclusion criteria,including 101 males and 115 females.The study subjects were divided into facet joint degeneration group and non-facet joint degeneration group according to different segments.MRI images were used to diagnose facet joint degeneration,and Image J software was used to measure the percentage of fat infiltration of paraspinal muscles(multifidus,erector spinae,psoas major).The statistical software SPSS25 was used to analyze the data.Result:1.In the three lumbar spine segments,there is no statistically significant difference in the asymmetry degeneration of the left and right facet joints(P> 0.05),and the facet joint degeneration is symmetrical;in the three lumbar spine segments,the left and right paravertebral muscles((Multifidus,erector spinae,psoas major muscle)fat infiltration asymmetry was not statistically significant(P> 0.05).2.In the three lumbar spine segments,the age composition of facet joint degeneration group and non-facet joint degeneration group is significantly different(P <0.05);in the three lumbar segment,facet joint degeneration group and no facet joint degeneration There was a significant difference in the degree of fat infiltration of paraspinal muscles(multifidus,erector spinae,and psoas major)in the variable group(P <0.05);in the three lumbar segments,the prevalence of facet joint degeneration was between genders There was no significant difference(P> 0.05).3.Multivariate Logistic regression analysis results: in the three lumbar spine segments,the increase in the degree of multifidus muscle fat infiltration is a risk factor for facet joint degeneration(P <0.05);in the L4/ 5 segment,the degree of fat infiltration of the erector spinae Aggravation is a risk factor for facet joint degeneration(P <0.05);inthree lumbar segments,age increase is a risk factor for facet joint degeneration(P <0.05);in three lumbar segments,lumbar muscle fat infiltration Degree is not a risk factor for facet joint degeneration(P>0.05).Conclusion:In the physical examination population of 20-50 years old,in the three segments of the lower lumbar spine,the increased degree of multifidus fat infiltration is a risk factor for facet joint degeneration;In the physical examination population of 20-50 years old,in the L4 / 5segment,the vertical spine The increased degree of muscle fat infiltration is a risk factor for facet joint degeneration.
