| ObjectiveThis study was based on "structure-process-result" theoretical model.We built a scientific,sensitive,practical and unified sensitive quality index system suitable for intravenous infusion safety quality control in PIVAS through Delphi expert consultation method.Applying the quality control sensitive index to PIVAS intravenous infusion quality safety survey,we found that there are problems in PIVAS intravenous infusion quality safety,to provide scientific basis for mutual communication and learning of each PIVAS.Methods(1)Based on the theoretical model of "structure-process-result",this study preliminarily constructed the prototype of safety and quality sensitive index of PIVAS intravenous infusion through literature review.(2)Through the summary and statistics of two rounds of Delphi expert advice,the evaluation index was selected,and the enthusiasm,authority,opinion concentration and coordination degree of experts were analyzed to form the sensitive index of safety quality of intravenous infusion in PIVAS.(3)The weight coefficient of each index in the evaluation system were calculated by using AHP and Yaahp software.(4)5 PIVAS in Qingdao were selected as the evaluation object by convenient sampling method,The work quality of PIVAS were evaluated by the established evaluation index.Results(1)After two rounds of Delphi expert consultation,the results of this study are as follows: the positive coefficient of experts were 86.7% and 100% respectively.The authority degree of experts were 0.880 and 0.905 respectively.The coordination coefficient of expert opinions were 0.360 and 0.493 respectively,and P is less than 0.001.(2)Index consultation results: through literature review,the rudiments of safety and quality sensitive indexes of intravenous infusion in PIVAS preliminarily formulated include 3 of first level indexes,13 of second level indexes and 36 third level indexes.The first round of expert consultation results: according to the results of the first round ofletters,2 of third level structural indexes were deleted.11 of third level indexes of process indexes were integrated into 4,forming the second round of expert consultation questionnaire includes 1 of third levelindicators,13 of second level indicators and 27 of third level indicators.The second round of expert consultation results: according to the results of the second round of correspondence,the name of 1 of third level indicator was modified,and the notes on the calculation formula were added to the expert consultation,finally forming the safety and quality sensitive indicators of intravenous infusion in PIVAS,including 3 of first level indicator,13 of second indicators and 27 of third indicators.(3)The weight of each index were calculated by using AHP and Yaahp software.The results of the first level index: structure index is 0.1429,process index is 0.4286,result index is 0.4286.(4)Based on the constructed evaluation indexes,5 PIVAS in Qingdao were investigated.The results showed that: the daily workload of 5 PIVAS were quite different.The nursing staff in the PIVAS had the problems of low educational background and short working life.The "correct rate of sterile operation of drug dispensing" was 94.78%-96.94%.The training and assessment methods of each PIVAS were single,and PIVAS lacked scientific and comprehensive training and system assessment.The qualified rate of "validity period and batch number of drugs" and "consistency rate of accounts and materials" in some PIVAS were low,and the wrong rate of sub packaging in departments was high.The satisfaction of clinical departments with "drug delivery time" in PIVAS was generally low.The satisfaction of staff in PIVAS with "work pressure and environment" and "proportion of salary and payment" were low.Conclusion:Based on the mature theoretical model of "structure process result",the safety and quality sensitive index of intravenous infusion in PIVAS constructed adopting Delphi expert consultation method were scientific,reasonable,and comprehensive.The weight setting results of each evaluation index calculated adopting Delphi expert consultation method combined with AHP were scientific and reliability.The safety and quality sensitive indicators of intravenous infusion in PIVAS helped to find problems in management.The construction of safety and quality sensitive idexes of PIVAS provided reference tools for promoting PIVAS quality monitoring and continuous improvement. |
