Objectives1.To explore the clinical effect of Vitamin E Cream,Sesame Oil,Saifurun to prevent phlebitis caused by short-term(<6days)infusion of nutrient solution via intravenous indwelling needle.2.By comprehensively comparing the clinical effects,safety and expenditure of three external medicines to prevent phlebitis caused by short-term(<6 days)infusion of nutrition via intravenous indwelling needles,the prevention effect is selected to be good,simple,economical,and in line with the public demand for the best topical drugs,to provide reference for clinical medicine.MethodsIn this study,based on consulting a large number of domestic and foreign literatures,specific intervention plans were formulated by using group discussions and preliminary experiments.In this study,a randomized controlled study method was used to select 132short-term(<6 days)infusion of nutrient solution(including Compound Amino Acid,50% Glucose Injection,10% Potassium Chloride injection,Medium and Long Chain Fatty Acid)via intravenous indwelling needle.The patients were divided into three groups according to the random number table: Compound Vitamin E Cream group,Sesame Oil(Cuizi Brand Xiaomoxiang Oil)group,and Saifun(Liquid Dressing)group,with 44 cases in each group.The three groups of patients were punctured with indwelling needles by the responsible nurses,and the topical drugs were given before the first infusion,up to 10-15 cm along the vein,down to the close-heart-edge of the apply(1.5-3cm from the puncture point),and both ends to the back or the edge of the arm,gently massage for 3 minutes and keep the medicine for one hour,3 times a day,until the study stops.If the patient develops local or systemic symptoms such as redness,swelling,heat,pain,extravasation,the drug is discontinued.Double observation of the patient’s medication site: presence or absence of phlebitis,phlebitis score level,range,pain score,phlebitis occurrence time,skin and systemic reactions,etc.and the observation will be recorded on the study observation record form.Using SPSS20.0 software to analyze the data.Results1.Comparison of the three groups of patients’ general information dataThe comparing of the general information of the three groups of patients: age,sex,disease,vascular condition,position of indwelling needle,nutrition solution formula,nutrition solution osmotic pressure,infusion volume,etc.the difference was not statistically significant(P> 0.05)and was comparable.2.Comparison of clinical effects of three topical drugs in preventing phlebitis caused by nutrient solution2.1 Comparison of the incidence of phlebitis caused by nutrient solution in three groups of patientsThe incidence of phlebitis in the Vitamin E Cream Group was 43.18%,which in the Sesame Oil Group was 37.21%,and in the Saifurun Group was 76.19%.According to Chi-square Test,the incidence of phlebitis caused by nutrient solutions in the three groups was compared,and there was significant difference(P <0.01).Comparison between the two groups: Vitamin E Cream and Sesame Oil are superior to Saifurun in reducing the incidence of phlebitis caused by nutrient solution,and there was significant difference(P<0.01);but compared with Vitamin E Cream and Sesame Oil,the difference was not statistically significant(P> 0.05).2.2 Comparison of incidence of different scores of phlebitis caused by nutrient solution in three groups of patientsThere was a statistically significant difference in the incidence of phlebitis among different groups in the three groups(P <0.01).Multiple comparison results between the groups: the Compound Vitamin E Cream group and the Sesame Oil group were compared with Saifurun group,the difference was statistically significant(P <0.01);but the Compound Vitamin E Cream group compared with the Sesame Oil group,the difference was not statistical academic significance(P> 0.05).2.3 Comparison of visual range of phlebitis caused by nutrient solution in three groups of patientsThe range of phlebitis caused by short-term infusion of nutrient solution among the three groups was statistically significant(P <0.01).Comparison between the two groups:the difference between Compound Vitamin E Cream and Saifurun were statistically significant(P <0.01);but the difference between Compound Compound Vitamin E Cream and Sesame Oil was not significant(P> 0.05).2.4 Comparison of pain scores of phlebitis caused by nutrient solution in three groups of patientsThe scores of phlebitis pain caused by short-term infusion of nutrient solution among the three groups were statistically significant(P > 0.05).2.5 Comparison results of occurrence time of phlebitis caused by nutrient solution in three groups of patientsThere was a statistically significant difference in the occurrence time of phlebitis caused by short-term infusion of nutrient solution among the three groups(P <0.01).Comparison between the two groups: the Compound Vitamin E Cream group and the Sesame Oil group were compared with the Saifurun group,the difference was statistically significant(P <0.01);but the Compound Vitamin E Cream group and the Sesame Oil group were compared no statistical significance(P> 0.05).3.Evaluation results of adverse reactions of three external drugsIn the whole trial,no symptoms of skin and general discomfort appeared in the three groups.4.Comparison of prices and uses of three external medicinesThe median cost of prevention of phlebitis caused by nutrient solution in the three groups was 14.9 yuan in Vitamin E Cream group,13.9 yuan in Sesame Oil group,and 96 yuan in the Saifurun group.Comparing the costs of the three groups of patients,the difference was significant,P <0.01.Further pairwise comparisons were made: The Sesame Oil group was significantly different from the Compound Vitamin E Cream group and the Saifun group(P <0.01);while the compound Vitamin E Cream group and the Saifurun group was statistically significant(P <0.01).Conclusions1.Compound Vitamin E Cream and Sesame Oil can effectively reduce the incidence of phlebitis caused by short-term infusion of nutrient solution via intravenous indwelling needle,reduce the degree of venous injury,control the scope of phlebitis,reduce the pain caused by phlebitis,and delay the time of phlebitis.All can reduce the pain caused from phlebitis.2.Sesame Oil is a natural ingredient extracted from sesame seeds.It has outstanding preventive effects,no adverse reactions for external use,andIn terms of cost,the Sesame Oil group is superior to the other two groups,the cost is lower,the patients are more acceptable,and the clinical reference can be used.It shows the advantages of applying the traditional Chinese medicine concept of natural plant ingredients to medicine to prevent phlebitis caused by short-term infusion of nutrient solution.The excellent traditional Chinese medicine concept of the motherland should be applied to clinical nursing work to improve the level of nursing and provide patients with good effect,high safety,economical and convenient nursing services.
