Objective: To evaluate the classification of asthma symptom control in the Guidelines for the Diagnosis and Prevention of Bronchial Asthma in Children(2016 Edition),the test for respiratory and asthma control in kids(TRACK),fractional exhaled nitric oxide(FENO)and impulse moisturiser oscillometry lung function(IOS)for the control of asthma in children under the age of five.Methods: 100 children under 5 years old diagnosed with mild to moderate asthma were given inhaled corticosteroids(ICS)based drug atomization therapy for 3 month.These patients were followed up for 3 months and each month we got the condition of children using the classification of asthma symptom control in the Guidelines for the Diagnosis and Prevention of Bronchial Asthma in Children(2016 Edition).Each month we applied the test for respiratory and asthma control in kids.To compare the consistency of evaluation by using the guidelines for diagnosis and prevention of childhood bronchial asthma(2016 edition)and the TRACK.Compare TRACK values before treatment,1 month,2 months and 3 months after treatment.FeNO and IOS were tested once every 3 months.At the same time,we got the score of FeNO,IOS of each child.The score of FeNO,IOS,and TRACK were analyzed in pairs.At the same time,the results of exhaled nitric oxide value and pulse concussion lung function parameters were compared and analyzed before and 3 months after treatment.According to the classification of asthma symptom control in children in the guidelines for diagnosis and prevention of childhood bronchial asthma(2016 edition),100 children with asthma were divided into three groups: well-controlled,partially controlled,and uncontrolled groups.Whether there were statistical differences in TRACK,FeNO and IOS indexes among the three groups.According to TRACK,100 children with asthma were divided into well-controlled group and non-controlled group.The comparison of total respiratory resistance R5,peripheral respiratory resistance R5-R20,peripheral elastic resistance X5,central respiratory resistance R20 and FeNO between the two groups was statistically significant.Results :1.Children’s Asthma Symptom Control Level Classification and TRACK two asthma control evaluation methods have a high agreement on the results of asthma control in 100 children under 5 years of age(Kappa=0.667,P<0.001).2.followed up for 3months,each month TRACK value comparison has statistical significance(F =-13.479 ~-5.328,P<0.001).3.According to the "Children’s Bronchial Asthma Diagnosis and Prevention Guideline(2016 Edition)",100 children with asthma were divided into three groups.The difference in TRACK among the three groups was significant(H = 48.758,P<0.001),and the difference between the two groups within the TRACK group was significant(U =-3.132 ~-5.555,P<0.001);There was no statistically significant difference in Fe NO detection values between the three groups(H = 0.533,P>0.05);the three groups Comparison of IOS indicators,R5,R5-R20,X5,the difference was not different(H = 0.061 ~ 0.352,P>0.05);there was a statistically significant difference in R20 between 3 groups of asthma children(H = 8.671,P<0.05),of which there was no statistically significant difference between some control groups and well-controlled groups(U =-0.496,P>0.05),the results of uncontrolled group and partial control(U =-2.549,P<0.05),uncontrolled group and The well-controlled group(U =-2.834,P<0.05)had significant differences.4.FeNO,IOS,TRACK three asthma symptom detection methods were compared two by two correlations,FeNO and R5 had a weak negative correlation(P<0.05),FeNO and R5-R20,X5,TRACK have no correlation(P>0.05);TRACK and R20 have a weak negative correlation(P<0.05),while TRACK has no significant correlation with R5-R20,X5,FeNO(P>0.05).5.The results of two kinds of FeNO were positively correlated before and after 3 months of treatment with nebulized inhaled hormone(r=0.648,P<0.001).Correlation analysis between the two IOS parameters,before and after 3 months of treatment with nebulized inhaled hormone,the difference between R5,R20,R5-R20,X5 was statistically significant(P<0.05).6.According to TRACK,100 children with asthma were divided into two groups.There was no significant difference in R5,R5-R20,X5,and FeNO between two groups of asthmatic children(U =-0.100 ~-0.532,P>0.05).Compared with the uncontrolled group,the asthmatic children’s R20 was statistically significant(U =-0.820,P<0.05).Conclusion:1.TRACK and children’s asthma control level classification have a higher consistency in the assessment of asthma symptom control level in children under 5 years old.2.The combination of IOS and FeNO can make the assessment of symptoms in children with asthma more accurate.
