Objectives:To systematically assess the efficacy and safety of Tongfu method combined with mechanical ventilation in the treatment of COPD complicated with respiratory failure.Methods : Chinese databases(CNKI,VIP,Wanfang,CBM)and English databases(CENTRAL,MEDLINE,EMbase)were searched to retrieve randomized controlled trials about traditional Chinese medicine of Tongfu method combined with mechanical ventilation for COPD patients with respiratory failure.The retrieval time limit was from the inception of the databases to February,2020.Two assessors independently screened the literature and extracted the information.And then risk of bias was evaluated and meta-analysis was performed by using the Revman5.3 software.We used risk RR for categorical data;and MD or SMD for continuous data.Results:506 related trials were searched out and finally 23 were included in,which involved 1667 patients.The main results are as follows:1.Traditional Chinese medicine of Tongfu combined with mechanical ventilation can reduce the intubation rate and mortality of patients with chronic obstructive pulmonary disease and respiratory failure,reduce the mechanical ventilation time and infection control time,improve the success rate of weaning,improve respiratory mechanics index,blood gas index,immune index(CD4 + T cell,CD4 + T cell / CD8 + T cell),inflammation index and reduce the evaluation of APACHE-II to reduce the incidence of mechanical ventilation related adverse reactions(abdominal distention,Ventilator-associated phenumonia).2.There were no significant differences in the effect of Tongfu method on CD8 + T cells quantity.3.It is safe to use Tongfu method in COPD with respiratory failure.Conclusions : Traditional Chinese medicine of Tongfu combined with mechanical ventilation has better efficacy and safety for COPD complicated with respiratory failure.And it is worthy of clinical promotion.Limited by the quantity and quality of the included studies,the conclusions need to be further verified with more high-quality clinical studies.
