ObjectiveThis study is aimed to respectively measure and compare the Oxygenated Hemoglobin(Oxy-Hb),Deoxygenated Hemoglobin(Oxy-Hb)and Deoxygenated Hemoglobin(Deoxy-Hb)in prefrontal area of brain during cognitive activities process in alcohol dependent patients and healthy drinkers,so as to find the change of blood oxygen level in prefrontal lobe during the cognitive activities of alcohol dependent patients,at the same time,respectively evaluate the cognitive function of the two groups to study the relationship between the cognitive function of alcohol dependent patients and the relative concentration level of Oxy-Hb,Deoxy-Hb in prefrontal lobe,and to explore the predictive value of the relative concentrations of Oxy-Hb and Deoxy-Hb in the prefrontal area on the cognitive function impairment of alcohol dependent patients,so as to provide help for the clinical treatment of alcohol dependent patients.MethodsTake the alcohol dependent patients in the substance dependence ward of mental health center in Inner Mongolia Autonomous Region as the research object in this study,the patients are required to stop the use of alcoholic drinks for at least 14 days and have been stopped using the sedative drugs of diazepam at present.Select the same number of healthy adults as the control group,and the basic information(gender,age,nation,years of education)is required to match the alcohol dependence group.Assess the cognitive function of two groups,including the Wisconsin Card Sorting Test(WCST)evaluation execution function;Brief Visuospatial Memory Test Revised(BVMT-R),Digital Sequence(DS)evaluation memory function;Digital Span(DS),Continuous Performance Test(CPT)evaluation attention function;after the completion of related evaluation,measure the relative concentration of Oxy-Hb and Deoxy-Hb in prefrontal lobe under the two cognitive activities by using ETG-4000 brain function quantitative imaging device,and carry out the correlation analysis of cognitive function evaluation results.Result1 Performance evaluation results show:the total response score in the Wisconsin card sorting test of alcohol dependence group(t=-13.681,P<0.001),the response score to complete the first sorting(t=-21.3,P<0.001),the continuous response score(t=-8.573,P<0.001)and the continuous error score(t=-12.121,P<0.001)are higher than that in the healthy control group,the correct response score(t=22.172,P<0.001),the completed sorting score(t=22.672,P<0.001)are lower than the healthy control group;memory function test results show:the score of brief visuospatial memory test revised(t=27.025,P<0.001)and digital sequence test(t=17.84,P<0.001)are significantly lower than those of the healthy control group;pay attention to the function evaluation results:the scores of digit span test(t=14.804,P<0.001),continuous performance test(t=9.039,P<0.001)are significantly lower than those of the healthy control group.The above difference is statistically significant(P<0.05).2 The concentration level of Oxy-Hb of prefrontal lobe is increased in both alcohol dependent group and healthy control group,but the relative concentration in alcohol dependent group is lower than that of healthy control group(t=13.026,P<0.001);the concentration of Deoxy-Hb in prefrontal lobe is decreased but the relative concentration is lower than that of healthy control group(t=-3.490,P=0.001),the difference is statistically significant(P<0.05).3 Performance evaluation results:the total response score in the Wisconsin card sorting test of alcohol dependence group(r=-0.707,P<0.001),the response score to complete the first sorting(r=-0.705,P<0.001),the continuous response score(r=-0.198,P<0.001)and the continuous error score(r=-0.646,P<0.001)are negatively correlated with the relative concentration level of Oxy-Hb,the correct response score(r=0.536,P<0.001)is positively correlated with the completed sorting score(r=0.298,P=0.001)and relative concentration level of Oxy-Hb,memory function test results:the score of brief visuospatial memory test revised(r=0.613,P<0.001)and the score of digital sequence test(r=0.450,P<0.001)are positively correlated with the relative concentration level of Oxy-Hb;pay attention to function evaluation results:the score of digit span test(r=0.376,P=0.003),the score of continuous performance test(r=0.583,P<0.001)and Oxy-Hb relative concentration level are positively correlated.There is no significant correlation between the scores of cognitive function and Deoxy-Hb of prefrontal lobe in alcohol dependence group4 The ROC curve analysis of the relative concentration level of Oxy-Hb in prefrontal lobe of alcohol dependence group shows that:Oxy-HB ≤0.155 can be diagnosis of alcohol dependence cognitive dysfunction,Oxy-Hb>0.155 can be diagnosis of non-alcohol dependence cognitive dysfunction,these are the highest diagnostic accuracy(P<0.05)Conclusion1 There are obvious dysfunction of executive function,attention function and memory function in alcohol dependent patients2 The relative concentration of Oxy-Hb and Deoxy-Hb in prefrontal lobe was significantly lower than that of normal people,and the relative concentration of Oxy-Hb was significantly higher than that of Deoxy-Hb.Oxy-Hb can be used as a physiological index to reflect the change of regional cerebral oxygen level in cognitive activities3 The executive function,memory function and attention function of alcohol dependent patients are positively correlated with the relative concentration of Oxy-Hb in prefrontal lobe The measurement of Oxy-Hb in prefrontal lobe can reflect the severity degree of cognitive impairment in alcohol dependent patients,and provide reference for the prediction of cognitive impairment in alcohol dependent patients4 The level of Oxy-Hb and Deoxy-Hb in prefrontal lobe can predict the cognitive impairment of alcohol dependent patients,which is worthy of clinical application.
