Study On The Regularity Of Traditional Chinese Medicine Use And Related Factors Of AECOPD(Syndrome Of External Cold And Internal Drinking)

Objictive:Summarize and analyze the basic situation,inflammatory index,blood gas analysis,symptom characteristics and medication of patients with AECOPD（external cold and internal drinking syndrome）on the day of admission,so as to find out the relevant rules and new prescriptions,so as to provide reference for clinical practice.Methods:The medical records of patients diagnosed as AECOPD（syndrome of external cold and internal drinking）were collected retrospectively,and the data were processed and analyzed by using the software of"Auxiliary platform for inheritance of Traditional Chinese Medicine（V2.5）"and Excel table.Results:1. There are more male than female hospitalized patients; the average age was72.51 years old;the average body mass index was 22.57kg/m²;137 cases（75.27%）were caused by exogenous pathogens;101 cases（55.49%）had a history of smoking.2. The average values of WBC,NEUT%,and CRP of the patients were 8.28×10⁹/L,72.91%,and 29.85mg/L,respectively,and the average values of Pa O₂and Pa CO₂were 70.54mm Hg and 41.90mm Hg,respectively.3. The main symptoms of the patients were cough,chest tightness,shortness of breath,white foamy sputum and aversion to cold; the tongue image was mainly light red tongue,red tongue and dark tongue;the fur was mainly white slippery fur,white greasy fur and thin white fur;the pulse was mainly stringed pulse and slippery pulse,among which there were 7 kinds of single pulse（19.23%）and 17 kinds of phase and pulse（80.77%）.4.The four qi of the drugs used are mainly warm,the five flavors are mainly pungent,and the meridians are mainly lung meridians.5.A total of 162 flavor drugs were used with a frequency of 2426 times,and 32drugs with frequency≥20 were used.6.Based on the analysis of the prescription rule based on entropy clustering,three new prescriptions are obtained:new prescription 1.Bupleurum,Schisandra Chinensis,Schisandra Chinensis,Fructus Schisandrae,Herba Ephedrae,Asarum,Dried Ginger;new prescription 2.Codonopsis Pilosula,Scutellaria,Bupleurum,Tangerine Peel,Schisandra,Asarum,Dried ginger.New prescription 3.Albizzia Bark,Yu Li Ren,Hemp Kernel,Melon Betel,Cyperus Roxburghii.Conclusions:1.AECOPD（syndrome of external cold and internal drinking）often occurs in elderly patients,and the prevalence rate of male is higher than that of female.Low body weight and normal weight are the main causes of the disease.External pathogen is the main cause of acute exacerbation of the disease,and smoke and dust play an important role in the disease.2.The reason why the average value of WBC and NEUT%is within the normal range and the average value of CRP does not increase obviously on the day of admission of patients with AECOPD（syndrome of external cold and internal drinking）may be related to the low immunity caused by the age of onset and the degree of inflammatory reaction in the early stage of the disease.The reason why AECOPD（syndrome of external cold and internal drinking）patients are less complicated with respiratory failure on the day of admission may be related to the fact that the disease is still in its early stage.The above two may also be related to the small sample size.3.Although the pathogenesis of AECOPD（syndrome of external cold and internal drinking）is external cold and internal drinking,but it can be combined with phlegm,heat,weakness of qi and blood,dampness,qi stagnation,blood stasis,food accumulation and so on.4. The traditional Chinese medicines of AECOPD（syndrome of external cold and internal drinking）are mainly pungent,warm and return to the lung meridian.5. The prescription of AECOPD（syndrome of external cold and internal drinking）in our hospital is mainly modified by Xiaoqinglong decoction and Erchen decoction.6. Three new prescriptions are obtained,which are applicable to syndrome of external cold and internal drinkin,syndrome of external cold internal drinking combined with Shaoyang,syndrome of external cold internal drinking combined with stagnation of intestinal qi.
