Randomized Controlled Clinical Study Of Acupuncture Sensitization Point In The Treatment Of Chronic Neck Pain

Objective: In this study,patients with chronic neck pain were taken as the research object,and different intervention measures were adopted,namely acupuncture sensitization point and non-acupuncture point,To observe the clinical efficacy of acupuncture sensitization point in the treatment of chronic neck pain,and to explore the relationship between thermal and pain sensitivity and the efficacy.so as to provide evidence-based evidence for the clinical application of sensitization point.Methods: In this study,114 patients with chronic neck pain were included and randomly divided into the experimental group(acupuncture treatment group,patients receive acupuncture at 5 sensitization points)and the control group(waiting group)by the central random system.The study period was 8 weeks,4 weeks at baseline and 4 weeks at treatment.Main evaluation index of NPQ neck pain scale,and the evaluation before and after the treatment,the secondary indicators for a week to have a number of days of neck pain,neck pain onset time every day,with infrared thermal imager and tenderness meter detecting 15 point tenderness,threshold and temperature measured once every two weeks,the treatment period were measured three times,the acupuncture prescription for each pressure measuring results of the most sensitive five points,total treatment 10 times.All the evaluation indexes were analyzed by the full analysis set.Results: 1.Baseline analysis: a total of 114 cases of chronic neck pain were completed in this study,including 56 cases in the acupuncture treatment group and 58 cases in the waiting treatment group,and 5 cases of shedding.There was no statistically significant difference in baseline status between the two groups at the time of enrollment,which was comparable(p>0.05).2.Efficacy analysis: the NPQ score of the acupuncture treatment group decreased from 37.79 points to 25.45 points,the number of weeks of onset of neck pain decreased from 6.35 days to 3.5 days per week,and the duration of daily onset of neck pain decreased from 6.98 hours to 3.62 hours,and the difference was statistically significant(p<0.05).There was no significant change in the waiting group,with no statistically significant difference(p>0.05).The number of weeks and duration of daily neck pain onset in the acupuncture treatment group were significantly improved compared with the waiting treatment group(p<0.01),and the NPQ score was decreased(p<0.05).3.Selection of sensitization points: the five most common sensitization points in the acupuncture treatment group were tianliao R,jianzhongshu L,tianliao L,da zhu R and da zhui.Sensitization points of acupuncture were mainly distributed in the two meridians of small intestine of hand taiyang and sanjiao of hand shaoyang.4.Comparison of tenderness threshold: there was no statistically significant difference between the three tenderness measurements at all points in the acupuncture treatment group(p>0.05),and no statistically significant difference between the three tenderness measurements at all points in the waiting treatment group(p>0.05).The threshold of three times of tenderness was compared between the acupuncture treatment group and the waiting treatment group,Only the Jian Jing L difference was statistically significant(p<0.05).5.Comparison of temperature values: there was no statistical significance between the three temperature measurements at all points in the acupuncture treatment group(p>0.05).The comparison of the three temperature measurements in the waiting treatment group showed that the third temperature measurement values of Tianzhu L,da zhui,Lieque L,Lieque R and Shousanli R were lower than the first temperature measurement values,and the second temperature measurement values of da zhui and manual Shousanli R were lower than the first temperature measurement values,with statistically significant differences(p<0.05).Three temperature measurements were compared between the two groups,and there was no statistically significant difference in all acupoints(p>0.05).Conclusions: 1.Acupuncture at sensitization point has a positive effect on chronic neck pain.After 4 weeks of enrollment,the NPQ score,the number of weeks of neck pain and the duration of daily neck pain were significantly improved in the acupuncture treatment group compared with the waiting treatment group.2.The five acupoints with the highest frequency of sensitization in the acupuncture treatment group were all distributed locally,and the acupoints of sensitization were mainly the shaoyang channels and the taiyang meridian of the hand.3.After acupuncture treatment,the condition of chronic neck pain improved.There was no significant change of tenderness threshold in the acupuncture treatment group,and no significant change in the waiting treatment group,which may be related to the decreased sensitivity of pain perception in patients with chronic neck pain.There was no significant change in the temperature of the acupuncture treatment group,while the temperature of the waiting treatment group showed a decreasing trend,suggesting that the temperature tended to be stabilized by acupuncture sensitization points.
