| Objective: In many malignant tumors(including NSCLC),inflammatory response and its related blood markers are closely related to the curative effect,prognosis and survival rate.Among these markers,Neutrophil-Lymphocyte Ratio(NLR)and Platelet-Lymphocyte Ratio(PLR)have important clinical applications.The purpose of this study is to further clarify the correlation between NLR,PLR and curative effect of patients with advanced stage(IIIB,IV)NSCLC,and try to use the combination of NLR and PLR(NLR-PLR score)as a predictive indicator of the efficacy response and prognosis of cancer patients,and evaluate its clinical value in predicting the efficacy response and prognosis of patients with advanced NSCLC.Methods:1.We conducted a single-center retrospective study and analysis of 175 patients with advanced(IIIB,IV)NSCLC who had been diagnosed and received first-line treatment.2.According to the results of efficacy response evaluation,175 patients were divided into PD and non-PD groups.The prognosis of the patients was determined through the follow-up data,and the overall survival period was calculated3.Hematological examination results of patients in the two groups(PD group and non-PD group)during hospitalization were collected,and the NLR and PLR levels of patients in the two groups before treatment were respectively counted and compared,and the correlation between NLR,PLR and efficacy response was verified4.The NLR-PLR score was constructed by combining NLR and PLR,and the NLR-PLR scoring range were defined from 0 to 2 points as follows: 2 points,high NLR(>2.461)and high PLR(>248.4);1 point,high NLR or high PLR;0 point,NLR and PLR were not high.5.To further evaluate and verify the predictive value of NLR-PLR score,we compared the correlation between efficacy response and prognosis among patients with different scores.Results: 1.Among the 175 patients with advanced NSCLC,the number of patients in PD group and non-PD group were 64 and 111 respectively.2.The mean NLR values of the two groups were 4.837±4.386 and 3.090±1.602 respectively.The mean values of PLR were 252.7±151.3 and 195.8±93.9 respectively.Compared with the non-PD group,the NLR and PLR levels of PD group were higher,and it was statistically significant.3.The comparison showed that the NLR-PLR score of PD group was significantly higher than that of non-PD group(P=0.0009),and the survival time of patients with high NLR-PLR score was shorter than that of patients with low score(P<0.0001).4.The P value less than 0.05 was statistically significant,so it could be confirmed that NLR,PLR and NLR-PLR scores were significantly correlated with the curative effect and prognosis of patients with advanced NSCLC.5.Single factor and multi-factor analysis showed that curative effect and NLR-PLR score were two independent influencing factors(P<0.0001,P=0.0392).Conclusions:1.The results confirmed the correlation between NLR,PLR,NLR-PLR scores and the efficacy,prognosis of patients with advanced(IIIB,IV)NSCLC.2.As effective blood predictors,NLR,PLR and NLR-PLR can be used to predict the efficacy and prognosis of patients with advanced NSCLC.3.In the future,these markers can be widely used in the clinical m anagement and treatment decision of tumor patients through further studies. |
