| Objective The increase of blood li Pid is the main Pathogenic factor of cardiovascular and cerebrovascular diseases.Statins Play an im Portant role in the treatment and Prevention of cardiovascular and cerebrovascular diseases.Although the efficacy and safety of statins have been obtained a PProved,the he Patotoxicity in adverse reactions is still a very worrying factor for clinicians.This to Pic is mainly discussed correlation of elevated transaminase with three different dosages of statins(atorvastatin,rosuvastatin,simvastatin)and its clinical efficacy,so as to Provide the basis for clinicians to use rational drugs to ensure the effectiveness and safety of clinical medication.Methods 1.180 Patients with atherosclerotic cardiovascular disease(ASCVD)who were hos Pitalized in the cardiovascular de Partment of a hos Pital were selected as the study object.The blood li Pid classification of the study object was in line with the ASCVD blood li Pid abnormality standard sti Pulated in the[Chinese adult blood li Pid abnormality Prevention and control guide(2016 Revision)],and the clinical data were collected;2.The morning fasting venous blood were collected one week before taking medicine(T0),the first week after taking medicine(T1),the fourth week after taking medicine(T2),the eighth week after taking medicine(T3)and the twelfth week after taking medicine(T4).The indexes of APT,AST and the li Pids(low density li Po Protein cholesterol(LDL-C),high density li Po Protein(HDL-C),triglycerides(TG),total cholesterol(TC))were recorded;3.Single factor analysis and multi Ple logistic regression model were used to study the correlation between the increase of transaminase and the dosage,age,gender,BMI,abdominal circumference,smoking history,drinking history,fatty liver disease history and diabetes history;4.Atorvastatin tablets(10mg/d,20mg/d,40mg/d),rosuvastatin tablets(5mgd/d,10mg/d,20mg/d),simvastatin tablets(10mg/d,20mg/d,40mg/d)were selected to treat ASCVD Patients with dysli Pidemia,and the effect of blood li Pid treatment was observed before and after medication;5.SPSS21.0 statistical software was used to Process the resulting data.Results 1.A total of 180 Patients were included in the study.Who were given different doses of atorvastatin,rosuvastatin and simvastatin.2.Among them,there were 12 Patients with elevated transaminase,there’s no the Patients of the 12 who had elevated transaminase more than 3 times of the normal value,and no Patients with liver damage during the treatment;2.Statistically found that Patients taking atorvastatin 40mg and simvastatin 40mg are more likely to have elevated transaminase,which generally occurs in the first week(T1)to the fourth week(T2)after medication.Associated factors about the rise of transaminase:age,gender,body mass index,abdominal circumference,drinking history,smoking history,liver disease history,diabetes history and drug dosage,After logistic regression analysis,we obtained the relevant factors that drinking history and fatty liver(P=0.001,OR=12.52;P=0.002,OR=12.75)had significant difference(P<0.05);3.The statistical results show that atorvastatin,rosuvastatin and simvastatin can significantly reduce the LDL-C of ASCVD Patients,meanwhile,atorvastatin has significant effect on TG,rosuvastatin and simvastatin have significant effect on TC.For controlling HDL-C index level,there was no difference among the three statins(P>0.05).In the as Pect of blood li Pid standard-reaching rate of ASCVD Patients,the blood li Pid standard-reaching rate of Patients taking atorvastatin 40mg and rosuvastatin 20mg was higher than that of other grou Ps(P<0.05),it’s statistically significant.Conclusion By analyzing and com Paring the correlative factors of three different dosages of statins and transaminase,in general,the efficacy of the three drugs is significant,and they have reliable safety.Atorvastatin,rosuvastatin and simvastatin have significant effect on reducing LDL-C index of ASCVD Patients.At the same time,atorvastatin has significant effect on reducing TG index,rosuvastatin and simvastatin have significant effect on reducing TC index.In terms of monitoring the blood li Pid standard-reaching rate of ASCVD Patients,the blood li Pid standard-reaching rate of Patients taking rosuvastatin 40mg and rosuvastatin20mg is higher than that of other grou Ps.However,when taking high-dose atorvastatin and simvastatin,we need to Pay attention to the transaminase index of the Patients.At the same time,if the Patients with history of drinking and fatty liver use statins,we must instruct the Patients to visit the hos Pital regularly and test the liver function index according to the doctor’s orders to ensure the safety and effectiveness of the Patients’medication. |
