Predictive Factors For Death In Children With Severe Adenovirus Pneumonia

Objective: To investigate the risk factors of death from severe adenovirus pneumonia(SAP)in children in intensive care unit(PICU).Methods: The clinical data of 68 children with SAP in PICU,children’s hospital of chongqing medical university were retrospectively analyzed,and they were divided into the death group and the survival group according to their condition.Multivariate logistic regression analysis was performed.Results: Among the 68 children with SAP,50 were male and 18 were female,46 were between 6 months and 2 years old,accounting for 67.6%.45 cases(66.2%)occurred in spring and summer.There were 61 cases(89.7%)with heat peak over 39℃ and 21 cases(30.9%)with fever over 2weeks.56 cases(82.4%)were infected with mixed other pathogens.Intrapulmonary and extrapulmonary complications of different severity were observed.There were 23 deaths with a CFR of 33.8%.Single-factor analysis showed that the rates of congenital heart disease,alanine transaminase(ALT)> 100U/L,acute respiratory distress syndrome(ARDS)and severe extrapulmonary complications were higher in the death group than in the survival group,and the differences were statistically significant(P < 0.05).Multivariate logistic regression showed that congenital heart disease,ARDS and severe extrapulmonary complications were independent risk factors for death(P < 0.05).Conclusion: Children with severe adenovirus pneumonia are mostly6 months to 2 years old in PICU,with high incidence in spring andsummer,high heat peak and long fever time,easy to be infected with other pathogens,prone to other systemic complications,and high fatality rate.When combined with congenital heart disease,ARDS and severe extrapulmonary complications,children with severe adenovirus pneumonia in PICU have an increased risk of death.
