| Objective: to compare the correlation of chronic atrophic gastritis between endoscopic diagnosis and pathological diagnosis,and to explore the methods to improve the coincidence rate of both.Methods: the 192 cases of chronic atrophic gastritis diagnosed by endoscopy with pathologic results were collected and compared to analyse the correlation and coincidence rate between endoscopic and pathological diagnosis,by studying all cases’ clinical data and comparing endoscopic manifestations and pathological diagnosis.Results: 56 cases were confirmed the chronic atrophic gastritis by pathological diagnosis in 192 cases who were diagnosed by gastroscopy with coincidence rate of 29.17%.The coincidence rate of endoscopy showed mucosa red and white,mucosa folds flattened or some mucosal vascular revealed,mucosa rough and or granular or nodular performance and all have was 33.68%、24.82%、32.84%、42.86%,respectively(P>0.05).The coincidence rate about biopsy location was antrum,gastric angle and gastric body of 25.87%,39.54% and 47.50%,respectively(P<0.05).The CAG coincidence rates of 1,2,3 and 4 biopsy specimens were 26.58%,34.48%,66.66% and 100.00%,respectively,among which the CAG coincidence rates of 1 biopsy specimen or more than 1(2-4)were not statistically significant(P > 0.05).The infection rate of HP in patients with chronic atrophic gastritis under microscope and pathological diagnosis was52.00%,and the infection rate of HP in non-chronic atrophic gastritis was45.55%,P >0.05.50 patients were pathologically diagnosed with chronic atrophic gastritis and underwent 13 C breath test.they were divided into A,B and C by microscopic manifestation,and the infection rates of HP were48.47%,37.50% and 29.17%,respectively,P>0.05.Conclusion: There are misdiagnosis and missed diagnosis of the chronic atrophic gastritis through endoscopic examination alone,the awareness of the diversified manifestations under endoscopy and the combination of endoscopy and pathology should improve the diagnosis rate of the chronic atrophic gastritis. |
