| PART Ⅰ DIFFERENTIAL DIAGNOSIS VALUE OF THE WHITE MATTER HYPERINTENSITY IN ALZHEIMER’S DISEASE AND VASCULAR DEMENTIAObjective:To compare the images of Magnetic resonance imaging(MRI)of Alzheimer’s disease(AD)and vascular dementia(VaD),and discuss the value of the white matter hyperintensities(WMH)in the differential diagnosis of AD and VaD.Methods:The clinical data MRI images and of 35 patients with VaD and 74 patients with AD were retrospectively analyzed.Volumes of different brain regions were measured by using AccuBrain~TMM system,and visual scoring scale was used to evaluate the WMH in all patients.We then compared the WMH between AD and VaD.efficiency of related WMH indexes were evaluated by receiver operating Characteristic(ROC)curves.Results:There were larger absolute and relative volume of WMH in VaD patients than that in AD patients,and the differences were statistically significant(P<0.05).There was statistically significant difference in the visual scoring for Deep WMH(DWMH)between AD and VaD(P=0.015).(2)The absolute volume of WMH did best in related WMH indexes to identify AD and VaD.Conclusion:There were significant differences in the severity of WMH between patients with VaD and AD.The WMH had great value for differential diagnostic of VaD and AD.PART Ⅱ DIFFERENTIAL DIAGNOSIS VALUE OF THE VOLUME OF BRAIN REGIONS IN ALZHEIMER’S DISEASE AND VASCULAR DEMENTIAObjective: To compare the images of Magnetic resonance imaging(MRI)of Alzheimer’s disease(AD)and vascular dementia(Va D),and discuss the value of the volumes of brain regions in the differential diagnosis of AD and Va D.Methods: The clinical data MRI images and of 35 patients with Va D and 74 patients with AD were retrospectively analyzed.Volumes of different brain regions were measured by using Accu Brain TM system.We then compared the volumes of brain regions between AD and Va D.Differential diagnosis of AD and Va D adopted two methods including the principal component analysis(PCA)and establishing model by logistic regression method,and efficiency of all indexes were evaluated by ROC curves.Results:(1)When compared with the Va D patients,the AD patients showed statistically significant lower volumes(P < 0.05)in total intracranial volume,brain parenchyma,gray matter,white matter,hippocampus,amygdala,hypothalamus,frontal lobe(left,right),occipital lobe(left,right),temporal lobe(left,right),parietal lobe(left,right),hippocampus(left,right),amygdala(left,right),hypothalamus(left,right).(2)PCA revealed five important indexes including brain parenchyma,frontal lobe(left),occipital lobe(left),temporal lobe(left),parietal lobe(left).The identification model consisting of brain parenchyma,amygdala,hypothalamus,occipital lobe(left)and age was established based on logistic regression analysis.(3)Among these indexes,the identification model had the best performance to differentiate between AD and Va D,and the sensitivity and the specificity were 70.3% and 80.0%,respectively.Conclusion: There were significant differences in the volumes of some brain regions between patients with Va D and AD.The volumes of brain regions had great value for differential diagnostic of Va D and AD. |
