Construction And Research Of Risk Assessment Index System For Airway Stenosis Caused By Adult Tracheal Intubation

Objective: At present,there is no clinical evaluation index system for the risk of airway stenosis caused by tracheal intubation in adults.Early prevention of airway stenosis,reduce the burden of patients and improve clinical outcomes,promote the quality and level of tracheal intubation care.Methods: The subject research identified the entry pool of risk factor index system for tracheal intubation caused by tracheal intubation through literature analysis and expert qualitative interview method,and initially established the risk assessment of adult tracheal intubation caused by tracheal intubation through Delphi expert correspondence Index system,and test its reliability and validity.Then,the analytic hierarchy process and Delphi expert consultation results are used to calculate weights and assign values,and finally form an adult airway intubation risk assessment index system.Results:(1)The risk evaluation indicators for adult airway intubation caused by tracheal intubation constructed in this study include 5 first-levelindicators(balloon factor,intubation factor,patient factor,comorbidity factor and drug use factor),24 secondary indexes 43 three-level indicators.(2)The effective recovery rates of the questionnaires in the two rounds of expert consultations were 92.60% and 100% respectively.In the first round of consultations,17 experts made suggestions and the rate of opinions was 63%.0.93 and 0.96,the expert authority(Cr)is 0.90 and 0.93respectively;the coordination coefficient of the two rounds of experts is0.15 and 0.16,and the coefficient of variation is 0.20 and 0.18 respectively.(3)The weight of each index was calculated by analytic hierarchy process.The consistency test of each level index was <0.1,among which the airbag factor was 0.2789,the intubation factor was 0.2605,the patient factor was 0.2478,the comorbidity factor was 0.1103,and the drug use factor was 0.1025.Conclusion:This study established an adult tracheal intubation-induced airway stenosis risk assessment index system that includes 5 first-level indicators,24 second-level indicators,and 43third-level indicators,providing a reference for the risk assessment of airway stenosis in patients with endotracheal intubation.Basis to achieve early prevention of airway stenosis,reduce patient burden and improve clinical outcomes,and further improve the quality and level of endotracheal intubation care.
