Objective: By evaluating the cerebrospinal fluid related TNF-α 、 MMP9 、 UCH-L1 inflammatory cytokines in neonatal purulent meningitis to study the correlation in the prognosis of the disease.Methods: From March 2015 to March 2017,68 cases of neonatal purulent meningitis treated in our hospital(observation group)and 62 cases of non-intracranial infections(control group)were studied.The levels of TNF-α,MMP9,and UCH-L1 in cerebrospinal fluid within 24 hours after admission were compared,and their correlation with prognosis of the disease was studied.12 months after birth,the Ge Sell scale and the brain MRI was used to evaluate the prognosis.Results: The levels of TNF-α,MMP-9,and UCH-L1 in cerebrospinal fluid were significantly higher in the purulent meningitis group than in the control group within 24 hours after admission.After discharge from the purulent meningitis group,62 patients were followed up and 6 were lost;in the purulent meningitis group,the levels of TNF-α,MMP-9,and UCH-L1 in the cerebrospinal fluid of DQ values < 86 group(16 cases,25.81%)were significantly higher than those in DQ values ≥86 group(46 cases,74.19%);the levels of TNF-α,MMP-9,and UCH-L1 in cerebrospinal fluid of the abnormal cranial MRI group(21 cases,33.87%)were significantly higher than those in the normal cranial MRI group(41 cases,66.13%);in the group with DQ values ≥ 86,39 cases(84.79%)were normal in cranial MRI,and 7 cases(15.21%)were abnormal;in the group with DQ values < 86,2 cases(12.50%)were normal in cranial MRI,and 14 cases(87.50%)were abnormal;the abnormal cranial MRI rate in the group with DQ values < 86 was significantly higher than that in the group with DQ values ≥ 86.There was a significant difference between the groups,P<0.01,with statistical significance;the higher the DQ value of the Ge Sell scale,the lower the abnormal rate of the cranial MRI.Conclusion: 1.The level of TNF-α,MMP9 and UCH-L1 in the cerebrospinal fluid of purulent meningitis were increased at the early stage of disease,which had certain reference value for early diagnosis of diseases.2.The level of TNF-α,MMP9 and UCH-L1 in the cerebrospinal fluid of purulent meningitis may be positively correlated with the severity of the disease,which may be negatively correlated with the prognosis,and may have a certain predictive value for the prognosis of the disease.
