The Current Influencing Factors And Mediating Effect Analysis Of Psychological Resilience In Patients With Hemopoietic Stem Cell Transplantation

Objective To investigate the influencing factors and export the path between the optimistic tendency,self-efficacy,medical coping style and social support with psychological resilience among patients with hemopoietic stem cell transplantation,provide the targeted intervention measures for subsequent conduction.Methods Cross-sectional survey was used in this study,From February 2017 to June 2018,Totally 228 patients with hemopoietic stem cell transplantation from the first affiliated hospital of USTC were investigated with General Social Demographic Information Questionnaire and Patient Disease Related Information,Revised Life Orientation Test(LOT-R),Resilience Scale(RS-14),General Self-Efficacy Scale(GSES),Medical Coping Mode Questionnaire(MCMQ)and Perceived Social Support Scale(PSSS).The data were processed by SPSS Software Package(Version 21.0).Statistical methods including descriptive analysis,single factor analysis,multiple linear regression analysis and Bootstrap were used in patients with hemopoietic stem cell transplantation.Results(1)Totally 250 patients with hemopoietic stem cell transplantation meeting the inclusion criteria were investigated,only 228 questionnaires were qualified,so the effective collect rate was 91.2%.(2)The average score of the participants was(73.04±13.41)points for resilience.There existed a difference in RS-14 scores among the patients with different occupational status,family income,survival time,the types of oral prescription drugs,KPS score of patients(all P<0.05).(3)The average score of the participants was(21.97±3.80)points for optimistic tendency,(25.92±6.75)points for self-efficacy,(19.91±3.72)points for facing coping style,(8.42±2.76)points for resignation coping style,(68.18±12.76)points for social support,Univariate analysis indicated that the RS-14 scores were positively correlated with LOT-R,GSES,MCMQ,PSSS,(r = 0.242～0.634,all P < 0.01);negatively correlated with resignation coping style(r =-0.382,P < 0.01);no correlation with avoiding coping style(r = 0.023,P>0.05).(4)Results of multivariate linear regression analysis showed that optimistic tendency,self-efficacy,facing coping style,social support,three kinds of prescription drugs are the influence factors of patients with hematopoietic stem cell transplantation.(5)Self-efficacy and social support moderates the relationship between optimistim and resilience.self-efficacy and social support mediation effect value were 0.96 and 0.23.and the mediation effect were 79.20% and 31.73%,No mediation effect of the medical coping styles.Conclusions(1)The level of psychological resilience was(73.04±13.41)points among the patients.(2)The optimistic tendency,self-efficacy,social support,three kinds of prescription drugs are the influence factors of hematopoietic stem cell transplantation patients.(3)Self-efficacy and social support moderates the relationship between optimistim and resilience.
