Objective:To retrospectively analyze the treatment of lumbar spinal canal stenosis in the elderly with percutaneous interstitial foraminal TESSYS technology,and to summarize its feasibility,safety and clinical efficacy.Methods: From September 2017 to September 2019,elderly patients with lumbar spinal stenosis were treated in our hospital’s orthopaedics department.According to the inclusion and exclusion criteria,a total of 50 eligible patients were selected.All patients were randomly divided into study groups according to the admission sequence number.Percutaneous intervertebral foraminal TESSYS technique was used,age was(66.23 ± 9.16)years,12 male patients,13 female patients,2 patients with L3-4 vertebrae,20 patients with L4-5,and L5-S1 There were 3 cases;25 cases in the control group underwent conventional open surgery for posterior lumbar interbody fusion,aged(67.16 ± 9.28)years old,14 males,11 females,1 with L3-4,and L4-5 There were 22 patients and 2 patients with L5-S1.The operation time,intraoperative blood loss,number of intraoperative fluoroscopy,length of hospital stay and postoperative complications,waist and leg pain VAS score,ODI score and MacNab efficacy evaluation were compared between the two groups.Results: The data was analyzed and processed by SPSS.19 software.The operation time of the TESSYS group was(60± 26)min,the intraoperative blood loss was(50 ± 15)ml,the number of intraoperative fluoroscopy(7 ± 3),and the hospitalization time was(3.35 ± 1.12)d.The number of postoperative complications(0)were better than those in the control group(116 ± 32)min,(400 ± 80)ml,(20 ± 4)times,(7.45 ± 1.89)d,(2)cases,P <0.05 was statistically significant;the VAS scores on the 1st day,1st week,3th month,and 6th month of the two groups were significantly lower than those in this group before surgery(P <0.05),and the patients in the TESSYS group The VAS scores were significantly lower than those of the open surgery group during the same period(P = 0.000).At 1,3,6 and 12 months after surgery,the ODI scores of the two groups were significantly lower than before surgery(P<0.05).The ODI scores at different time points were significantly lower than those in the open surgery group(P <0.05);the comparison of Macnab curative effects at 6 months after operation in the two groups was not statistically significant(P> 0.05).Conclusion: After grasping the strict indications and contraindications,the treatment ofelderly lumbar spinal stenosis with percutaneous intervertebral foramen TESSYS technology is a safe,minimally invasive,patient-acceptable and accurate clinical effect.Compared with posterior lumbar interbody fusion,the surgical trauma is less,does not damage the spine stability,can significantly shorten the length of hospitalization and surgery,reduce intraoperative fluoroscopy and bleeding,reduce pain and postoperative complications,and quickly recover The function of daily life is an effective and more advantageous solution for the treatment of lumbar spinal stenosis in the elderly.
