Objective: 1.To compare the level of the absolute number of lymphocyte subsets and CD4+T cell subsets between SS patients and HC,and futher discover the correlations between these lymphocyte subsets and disease activity measures such as ESR,CRP and ESSDAI scores.2.To compare the intestinal microbiota relative abundance diversity between SS patients and HC.3.To investigate the relative abundance of intestinal flora of SS patients with HC,and further analyze the correlation between these differences and the absolute number of Treg cells and Th17 cells.To explore the correlation between the characteristic flora and disease activity measures such as ESR,CRP and ESSDAI scores was analyzed.Methods: Outpatient and inpatient SS patients in the department of rheumatology of the Second Hospital of Shanxi Medical University from October 2018 to May 2019 were enrolled in this study,which include 29 SS patients(10.3% males and 89.7% females,with an average age of 47.17 ± 9.32 years)who met the American rheumatology SS classification standard revised in 2012.Those diagnosed with systemic sclerosis,RA,SLE and other autoimmune diseases or intestinal diseases were excluded.A total of 83 gender-and age-matched healthy adults(12% males and 88% females,with an average age of 49.19±9.71 years)from the health examination center the of SS patients were also enrolled as HC group.Peripheral venous blood and feces of the HC group and SS group above were collected,and the number of peripheral blood lymphocyte subsets was observed by flow cytometry.The V3 region of 16 S r RNA of the study subjects was qualitatively analyzed by Illumina Hiseq 2500 high-throughput sequencing platform.We analyzed the absolute number of peripheral blood lymphocyte subgroup and intestinal flora changes between the SS and HC group,and the intestinal flora diversity and differences in relative abundance, as well as the disease acitivity measures such as ESR,CRP and ESSDAI score.We also explored the change of lymphocyte and the relevance of differences between bacteria.Results: 1.Compared with the HC group,the absolute number of T cells,CD4+T cells,CD8+T cells and NK cells in the peripheral blood lymphocyte subsets were significantly decreased in SS group.There was also a significant decrease of the number of Th1 cells,Th2 cells,and Treg cells in the CD4+T cell subsets in the SS patients compared with those of HC.Treg cells in SS patients were negatively correlated with ESSDAI scores,while no significant correlations observed between other lymphocyte subsets and disease activity measures such as ESR,CRP,and ESSDAI scores.2.Compared with the HC group,the intestinal flora diversity of the SS group decreased,and the relative abundance of proteobacteria,actinomycetes,streptococcaceae,enterobacteriaceae,lactobacillaceae,Blautia,Oscillos Pira,streptococcus and Veillonella,was significantly increased.The relative abundance of firmicutes,prevotella,Para Prevotellaceae,Roseburia,Faecalibacterium and Prevotella decreased significantly.3.Compared with HC,the relative abundance of actinomycetes in the intestinal flora of SS patients was negatively correlated with the absolute number of Th2 cells.The relative abundance of streptococcaceae was negatively correlated with the absolute number of NK cells.The relative abundance of streptococcus was negatively correlated with the absolute number of Treg cells.The relative abundance of coprobacter was negatively correlated with the absolute number of Th17 cells.There was no correlation between the relative abundance of intestinal flora in the SS group and ESR,CRP and ESSDAI scores.Conclusion: Number of SS patients peripheral blood lymphocyte subsets changes involved in the progress of the disease,at the same time,SS in patients with intestinal micro bacteria community diversity and ecological environment balance exists obvious difference with the normal population and relative abundance of streptococcus rise and peripheral Treg cells absolutely count were significantly negative correlation,relative abundance of fecal bacteria genera and Th17 cells absolutely count were significantly negative correlation.It is suggested that the imbalance of intestinal microorganisms is related to the disorder of immune cells,which leads to the occurrence of SS diseases.These observations could provide a new direction for the study of the pathogenesis of SS,and provide a very meaningful clinical basis for the application of probiotics and their metabolites in the treatment of SS and even autoimmune diseases.
