Objective:Analyze the correlation and clinical predictive value of body measurement index(BMI),waist circumference(WC),waist-height ratio(WHt R)and abnormal glucose and lipid metabolism in polycystic ovary syndrome(PCOS).Methods:A retrospective analysis of 754 cases of PCOS patients of childbearing age,and the correlation between BMI,WC,WHt R and blood glucose and blood lipid indicators.Comparison of different anthropometric indexes of glucose metabolism abnormalities(including Impaired sugar regulation,Type 2 diabetes),lipid metabolism abnormality working characteristics(ROC)curves and their best cut-off points.Multiple comparisons of performance analysis indicators of anthropometric indexes.Logistic stepwise regression analysis of predictive value of anthropometric indexes for PCOS combined with abnormal glucose or lipid metabolism.Results:The age,BMI,WC,WHt R,FPG,2h PG,TC,TG,LDL-C of patients with abnormal PCOS glucose metabolism are higher than those with normal PCOS glucose metabolism,and the HDL-C of PCOS abnormal glucose metabolism patients is lower than that of patients with normal glucose metabolism(all P <0.05);the BMI,WC,WHt R,FPG,2h PG and TG of patients with abnormal PCOS lipid metabolism are higher than those with normal PCOS lipid metabolism,and the HDL-C of patients with abnormal PCOS lipid metabolism is lower than those with normal lipid metabolism(all P <0.05).WC,BMI,and WHt R are all closely related to blood glucose and lipid parameters(partial correlation coefficient 0.073-0.370,all P <0.05),and are not affected by age.The ROC-AUC of IGR and T2 DM have the largest WHt R(0.714,0.736),followed by WC(0.707,0.721),and BMI(0.679,0.692).The best cut-off points of the ROC curves for IGR and T2 DM are 0.55 and 0.57 respectively;WC are 88 cm and 90cm;BMI are 26.0kg /m2 and 28.0kg / m2.WHt R recognizes that PCOS combined with IGR have higher specificity,Yoden index,and coincidence rate than BMI,and the difference is statistically significant(P<0.0167).BMI,WC,WHt R recognizes PCOS combined with T2 DM show no significant difference in sensitivity,specificity,Yoden index,coincidence rate,positive likelihood ratio,negative likelihood ratio,positive predictive value,negative predictive value(P> 0.05).WHt R and age are significant in the binary logistic regression model.The risk of abnormal glucose metabolism in PCOS patients with WHt R ? 0.55 is 4.340 times that of WHt R <0.5;the risk of abnormal glucose metabolism in PCOS patients aged ? 30 years old is 2.343 times that of age <30 years old.The largest ROC-AUC of PCOS with abnormal lipid metabolism is WHt R(0.676),followed by WC(0.674)and BMI(0.670);The best cut-off points of the ROC curve recognized by WHt R for PCOS combined with abnormal lipid metabolism is 0.50,and WC is 82 cm,and BMI is 25.0 kg / m2.The sensitivity of WHt R is higher than BMI,and the negative likelihood ratio of WHt R is lower than BMI,the difference is statistically significant(P <0.0167).WC and WHt R are significant in the binary logistic regression model.The risk of abnormal lipid metabolism in PCOS patients with WC ? 82 cm is 2.303 times that of WC <82cm;the risk of abnormal lipid metabolism in PCOS patients with WHt R ? 0.5 is 2.162 times that for WHt R <0.5.Conclusion:Anthropometric indexes are closely related to PCOS glucose and lipid metabolism indicators.Anthropometric indexes can identify abnormal PCOS glucose and lipid metabolism.WHt R is the primary index for screening PCOS for abnormal glucose metabolism.Age and WHt R ? 0.55 are the risk factors for PCOS with abnormal glucose metabolism;WC ? 82 cm and / or WHt R ? 0.5 are the risk factors for PCOS with abnormal lipid metabolism,and both can be used as the primary screening index.
