Objective:1.Develop a simple screening tool that can quickly and effectively predict LVDS patients.2.Evaluate the actual use value of the tool.Objects and Methods:1.The general data and clinical manifestations of 314 patients with acute ischemic stroke between November 2017 and May 2018 were retrospectively collected.These patients received carotid ultrasound and cranial MRA examination after admission.Objective clinical manifestations such as consciousness disorder,gaze paralysis,advanced intelligence disorder,limb paralysis,speech disorder,facial paralysis,tongue paralysis and pap were used as primary indicators to screen patients with severe cerebral vascular stenosis or occlusive acute ischemic death.Patients were divided into LVDS group and non-lvds group according to whether they had severe cerebral vascular stenosis or occlusion.The influence coefficient of each indicator on LVDS prevalence(weight coefficient)was calculated,and the screening indicator was selected based on clinical practice.Calculate the weight coefficient of screening indicators,determine the score,and make screening tools.2.The screening tool was used to score 693 patients with acute ischemic stroke diagnosed with complete cerebrovascular data after admission to our hospital from May2018 to May 2019.The patients were divided into LVDS group and non-lvds group according to the vascular data.The AUC of the area under the curve of the screening tool was obtained by using the working characteristic curve of the subject.The optimal truncation value of the screening tool was calculated by using the truncation figure(to judge the criteria of LVDS patients).Evaluate the value of the screening tool in practicalapplication.Results:1.The primary index weight coefficients of 314 patients were included: consciousness disorder 3.33,gaze paralysis 2.88,advanced intelligence disorder 2.49,paralysis of the limbs 1.32,speech disorder 1.17,facial paralysis 1.4502372,tongue paralysis 1.17,pap sign 1.32.Screening index weight coefficient: consciousness disorder 3.38,gaze paralysis3.23,advanced intelligence disorder 2.42,paralysis 1.62,speech disorder 1.35.The cgc-fast scale of screening tool was made.2.Among the 693 patients included for evaluation,30% were patients with acute ischemic stroke due to vascular degeneration.The Youden index was the highest(0.64)when the optimal cut-off value of the gc-FAST screening tool was ≥3,that is,the tool judged the LVDS score ≥3,and the sensitivity,specificity,positive predictive value and negative predictive value were 0.69,0.95,0.87 and 0.87,respectively.The AUC and accuracy of gc-FAST scale were 0.84 and 0.73 respectively.Conclusion:The cgc-fast scale can be used as a rapid and effective screening tool to identify LVDS patients.
