Objective:Many studies have shown that patients with chronic constipation exist the alteration of intestinal flora.However,many discrepancies exist in the findings,and no clear link has been confirmed between chronic constipation and changes in the gut microbiota.Growing evidence indicates that age,gender,and hormone levels can affect the composition of gut microbiota.The relationship between constipation and gut flora is more intuitive in female patients of childbearing age due to their hormone level and intestinal flora were relatively stable,and often have no complications.Also,It is more likely to develop into refractory constipation,because of the long-term harm.Therefore,the aim of this study is to identify the overall change of fecal flora in female patients with chronic constipation of childbearing age,and then laying a foundation for improving the understanding of the pathogenesis of chronic constipation in specific populations.Methods:1 According to the Roman diagnostic criteria Ⅳ of functional constipation,the patients with chronic constipation and the healthy female volunteers of reproductive age were recruited as the case group and control group.2 The clinical symptoms of the case group were evaluated by Bristol stool form scale and KESS constipation symptom rating scale,and the main constipation classification was analyzed based on the symptoms.3 PAC-QOL quality of life questionnaire was used to compare the quality of lifescores of the two groups.4 Fecal samples were collected from the case group and the control group,and detected using 16 S rRNA sequencing.Then the structural changes of gut macrobiota in two groups were compared and analyzed.Results:1 The clinical symptoms of female patients with chronic constipation in childbearing age are mainly manifested as the decrease in the number of defecation(>80%)and the lumpy or hard stools(>80%).2 Compared with the healthy control group,the PAC-QOL total score,all dimensions scores in the case group were significantly higher than those in the control group(P<0.05).3 Compared with the healthy control group,there was a significant increase in bacteroidetes in the fecal flora of the case group(P<0.05),and a significant decrease in enterobacteriaceae(P<0.d01),leading to the ratio of Firmicutes to Bacteroidetes reduced.Additionally,there is a significant decrease of Roseburia and Fusicatenibacter(P<0.01).Conclusion:1 Slow transit constipation was the main type in female patients with chronic constipation of childbearing age,which seriously affects th quality of their life.2 There are gut macrobiota disorders in female patients with chronic constipation of childbearing age,mainly manifested as the reduce in the ratio of F/B,and a significant increase in bacteroides and a significant decrease in enterobacteriaceae.Additionally,there was a significant decrease in butyrate-producing bacteria,such as Roseburia and Fusicatenibacter.
