Objective To analyze the clinical characteristics of 96 cases with Primary gastrointestinal lymphoma(PGIL),so as to improve the understanding of Primary gastrointestinal lymphoma and provide reference for the clinical diagnosis of this disease.Methods The clinical characteristics of 96 patients with PRIMARY GASTROINTESTINAL lymphoma admitted to the General Hospital of Ningxia Medical University from January 2011 to December 2018 were retrospectively analyzed.Results1.There were 96 PGIL patients in this group,including 58 males and 38 females,with a male to female ratio of 1.5:1.0.Age 4-80,the average age of 18.0 years,59.0 the onset age to the elderly(60)is the most common abdominal pain PGIL the most common clinical symptoms,a total of 55 cases,followed by abdominal distension,11 cases of abdominal bag piece of 7 cases of 7 cases of nausea and vomiting hematemesis black 5 patients stop scheduling 3 defecate anti acid gas 2 2 2 cases bloody diarrhea defecate number increase 1case was thin in 1 case.2.The most common multiple gastric lesions occurred in the stomach in 20 cases,17 cases of gastric antrum in 13 cases of single lesions in the stomach in 19 cases of main intestinal lesions in the ileocecal region,followed by 7 cases of ileum in 6 cases of colon in 6cases of multiple intestinal lesions in 5 cases.3.The most common complication was obstruction,mainly intestinal obstruction in 18cases,followed by intussusception in 9 cases and intestinal perforation in 2 cases.At least 1case of gastric retention and 1 case of gastric perforation with pyloric obstruction was diagnosed by digestive endoscopy biopsy in 63 cases.The diagnostic rate of 33 cases of digestive endoscopy confirmed by surgical biopsy was 91.1%in gastric lymphoma and 30%in intestinal lymphoma.4.The main clinical manifestations under endoscopy were ulcerative type(a total of 43cases(26 cases of single ulcer,17 cases of multiple ulcer),followed by erosion(8 cases),uplift(6 cases),and irregular neoorganisms(5 cases).In this study,all the 39 PGIL patients received HP detection,and 28 of the detection results were positive(19 cases of gastric lymphoma and 9 cases of intestinal lymphoma),with a positive detection rate of 71.8%.There were 19 cases of increased tumor markers,77 cases of no tumor markers,13 cases of autoimmune diseases,and 8 cases of family tumor history.5.All the 96 patients with PGIL were non-Hodgkin’s lymphoma and 93 patients with pathological type B cell origin.Diffuse large B-cell lymphoma was the main type in 74 patients,followed by marginal B-cell lymphoma in extranodal mucosa associated lymphoid tissue in 15 patients,peripheral T-cell type in 3 patients,Burkit lymphoma in 3 patients,and B-lymphoblastic leukemia/lymphoma in 1 non-special type.Ⅰ period of 25 cases,1 case wasⅡ,Ⅱ 1 period of 28 cases,Ⅱ phase 2,9 cases Ⅱ E period 15 cases,18 cases Ⅳperiod.Gastric lymphoma most Ⅰ in 19 cases,12 cases intestinal lymphoma most Ⅱ in phase 1.6.93 cases of treatment,single chemotherapy 28 cases,surgery combined chemotherapy in 26 cases,single surgical removal of the HP treatment of 20 cases,9 cases with combination chemotherapy,single anti HP treatment of 4 cases,only symptomatic treatment support in 3 patients,chemotherapy combined radiotherapy,HP adjuvant radiotherapy resistance,resistance to HP combined radiotherapy and chemotherapy in 1 case of each item,3 cases of confirmed after giving up treatment.Conclusion1.Primary gastrointestinal lymphoma has a large age distribution span and is more common in elderly men.2.The most common clinical manifestations of primary gastrointestinal lymphoma are abdominal pain and bloating.3.The stomach is the most common site of primary gastrointestinal lymphoma,and the endoscopic manifestations are mainly single gastric ulcer.4.Primary gastrointestinal lymphoma is mostly b-cell derived,and diffuse large B-cell lymphoma is the most common pathological type.
