Objective : To explore the efficacy of oral saffron combined with anti-VEGF in the treatment of wet age-related macular degeneration(wet age-related macular degeneration,wAMD)and its association with age-related macular degeneration risk genotypes. Methods : 45 patients(60 eyes)with wAMD were included. The diagnosis of wAMD was confirmed by comprehensive ophthalmological examination, including best corrected visual acuity,non-contact intraocular pressure, slit lamp examination, fundus photography, optical coherence tomography angiography(OCTA),fundus fluorescein angiography(FFA),indocyanine green angiography(ICGA)and so on.Collected venous blood samples of patients treated with saffron supplement, and the complement factor H(rs1061170)polymorphism was detected, and the therapeutic effect of patients with risk genotypes was analyzed. The samples were divided into two groups: experimental group(n = 22)and control group(n=23).experimental group(28 eyes)was treated with saffron andanti-vascular endothelial growth factor(Anti-VEGF).Control group(32eyes)was treated only with anti-VEGF.Compared visual acuity, retinal edema and the size of choroidal neovascularization(CNV)before treatment with after treatment, and analyzed the curative effect of saffron on risk genotypes. Results: After treated by oral saffron combined with anti-VEGF,the CNV size and thickness of macular fovea in the experimental group were significantly improved than those in the control group(P<0.05).The total effective rate of case group was 96.43%,while the total effective rate of control group was 84.38%,there was statistical significance between the two groups(P<0.05).The total effective rate was 95.83% in 20 cases(24eyes)with r different genotypes of wAMD.Conclusion: CFH(rs1061170)mutation increases the risk of wAMD.Oral saffron combined with anti-VEGF therapy has an auxiliary effect on the improvement of visual acuity, reduction of retinal edema and CNV regression in wAMD,and is also effective for wAMD with low-risk TT genotype.
